SS23. The Vascular Quality Initiative Carotid Artery Stenting Stroke/Death Risk Score
Carotid artery stenting (CAS) was introduced as an alternative carotid revascularization procedure in patients deemed high-risk for carotid endarterectomy. Although techniques and patient selection criteria have dramatically improved, CAS continues to have higher risk of stroke and death in comparison with carotid endarterectomy. Several risk factors are known to be associated with worse outcomes. Although knowledge of these independent factors is helpful, clinical decision making is further refined when these are considered in aggregate.

VESS19. Addition of Proximal Intervention to Carotid Endarterectomy Increases Risk of Stroke and Death in the Vascular Quality Initiative
Adding ipsilateral, retrograde endovascular intervention (IPE) to carotid endarterectomy (CEA) for treatment of tandem bifurcation and supra-aortic trunk disease is controversial. Some suggest this combined strategy (CEA + IPE) confers no risk over isolated CEA. Others disagree, reserving CEA + IPE for symptomatic patients. Using the Vascular Quality Initiative (VQI), this study assessed the effect of adding IPE to CEA on stroke and death risk. We further weighed CEA + IPE outcomes in the context of symptomatic status and Society for Vascular Surgery guidelines.

Quantification of aortic shagginess as a predictive factor of perioperative stroke and long-term prognosis after endovascular treatment of aortic arch disease
Thoracic endovascular aortic repair (TEVAR) with supra-aortic debranching has recently been introduced as a treatment option for aortic arch disease. Although less invasive than open repair, TEVAR is associated with a risk of perioperative embolic stroke due to dislodgment of atherosclerotic plaque debris, especially in individuals with atheromatous degeneration of the aortic lumen. It is difficult to determine atheroma vulnerability, and there is no current method for predicting the risk of atheroembolism.

Carotid Endarterectomy After Systemic Thrombolysis in Stroke Patients: A Retrospective Comparative Analysis
Vascular specialists are increasingly being requested to perform carotid endarterectomy (CEA) after intravenous thrombolysis (IVT) for stroke patients, raising concerns about hemorrhagic complications. Few case series and registry reports have assessed the question, focusing on comparison with symptomatic patients. The goal was to evaluate the hemorrhagic and overall outcomes of patients undergoing CEA after IVT and to compare them with a similar population.
