Imaging Negative Stroke: Diagnoses and Outcomes in Intravenous Tissue Plasminogen Activator–Treated Patients
Conclusions: Because most INS patients were found to have NNCI, which may represent either transient ischemic attack or aborted stroke, and there were no intracerebral hemorrhages in the INS group, our data support the safety of administering IV t-PA to all patients in whom acute ischemic stroke is clinically suspected. We have demonstrated that NNCI patients and stroke mimics are common, and future larger scale prospective studies are required to delineate the true frequencies of each and to evaluate differences in outcomes.

Intravenous Thrombolysis with Neuroprotective Therapy by Edaravone for Ischemic Stroke Patients Older than 80 Years of Age

Stroke Mimics: An Important Source of Bias in Acute Ischemic Stroke Research
We report the characteristics, frequency, and outcomes of stroke mimics treated at our institution. Using our data, we then explore how the inclusion of stroke mimics in stroke outcomes research may be an important source of bias. Methods: We retrospectively identified all patients treated with alteplase in our emergency department from August 2013 to December 2017 for suspected acute ischemic stroke.

Visual stroke symptom triage tool to reduce delays due to limited English proficiency in Emergency department stroke activations
Limited English proficiency (LEP) may contribute to delays in care and worse outcomes in patients hospitalized with stroke. Limited access to interpreter services leads to delays in triage, code activation, imaging evaluations, and treatment. At our Comprehensive Stroke Center, 20% of patients with stroke have a Chinese dialect as a primary language and LEP. We sought to assess differences in stroke activation process times and IV alteplase treatment (IVT) rates between English and LEP (Chinese dialect) patients before and after implementation of a visual stroke symptom (VSS) triage tool.

Hospital Revisits for Post-Ischemic Stroke Epilepsy after Acute Stroke Interventions
Improvements in acute stroke care, namely the use of intravenous alteplase (IV-tPA) and mechanical thrombectomy (MT), have led to a considerable increase in the proportion of patients who survive a first-time ischemic stroke.1 Despite demonstrated benefits of acute stroke interventions, their impact on long term complications is not yet clear. One such complication is post-ischemic stroke epilepsy (PISE). PISE is estimated to occur in 2 –6% of stroke survivors,2–4 and is associated with poor-quality of life and high mortality in young patients.

Conclusions: The excess hemorrhagic risk after standard-dose alteplase infusion into older Afro–Caribbean patients warrants further study to determine the possible role of cerebral microangiopathy and should be evaluated in different black populations.

Fatal Multiple Systemic Emboli after Intravenous Thrombolysis for Cardioembolic Stroke
Our objective is to present a case of fatal multiple systemic emboli after intravenous thrombolysis for cardioembolic stroke. A 64-year-old woman with atrial fibrillation was admitted for evaluation of sudden consciousness disturbance, right hemiplegia, and aphasia. Diffusion-weighted imaging showed no early ischemic changes of the brain, and magnetic resonance angiography (MRA) showed occlusion of the left middle cerebral artery (MCA). Thromboelastographic Changes in Patients Experiencing an Acute Ischemic Stroke and Receiving Alteplase
Conclusions: Our study suggests that thromboelastogram (TEG) is a useful tool for determining changes in the coagulation system of patients whom have received recombinant tissue plasminogen activator (rt-PA). Further study is needed to determine if TEG can be used to predict those patients who may be at higher risk of adverse events because of rt-PA.

Predicting Stroke Outcome Using Clinical- versus Imaging-based Scoring System
Several models to predict outcome in ischemic stroke patients receiving intravenous (i.v.) alteplase can be divided into clinical-based and imaging-based systems. Alberta Stroke Program Early Computed Tomography (CT) Score (ASPECTS) and Dense cerebral artery sign/early infarct signs on admission CT scan, prestroke modified Rankin Scale (mRS) score, Age, Glucose level at baseline, Onset-to-treatment time, and baseline National Institutes of Health Stroke Scale score (DRAGON) are typical imaging- and clinical-based scoring systems, respectively.

Edaravone with and without .6  Mg/Kg Alteplase within 4.5 Hours after Ischemic Stroke: A Prospective Cohort Study (PROTECT4.5)
Edaravone is widely used to treat acute ischemic stroke (AIS) within 24 hours of onset. We aimed to evaluate current edaravone treatment practices and the efficacy and safety of edaravone used with recombinant tissue plasminogen activator (tPA) in AIS patients within 4.5 hours of onset. The results were compared with those of the Safe Implementation of Treatments in Stroke-International Stroke Thrombolysis Registry (SITS-ISTR) study.

Thrombolytic Therapy of Acute Ischemic Stroke during Early Pregnancy
Thrombolytic treatment (recombinant tissue plasminogen activator [rt-PA]) has established efficacy in acute ischemic stroke, but pregnancy has been an exclusion criterion for all clinical trials that validated alteplase in acute stroke, so our knowledge about its use in this condition is limited.Herein we report the successful use of intravenous rt-PA thrombolysis, uncomplicated by neither hemorrhage development nor other complication in a woman who was 13 weeks pregnant with acute ischemic stroke.
