Autoimmune diseases, their pharmacological treatment and the cardiovascular system.
Abstract Cardiovascular system involvement is a frequent complication of autoimmune diseases (AD) such as systemic lupus erythematosus, scleroderma, rheumatoid arthritis, spondyloarthropaties or psoriatic arthritis. The most common forms of such involvement are pericarditis, myocarditis, accelerated atherosclerosis resulting in myocardial infarction or stroke, arrhythmias, conduction abnormalities or congestive heart failure. Some of these manifestations may be dramatic in their course and ultimately fatal. The treatment of AD may further affect the cardiovascular system and result in a lower quality of life, high...

Resting heart rate at hospital admission and its relation to hospital outcome in patients with heart failure.
CONCLUSIONS: In patients with HF, higher resting HR at hospital admission is associated with increased in-hospital mortality. PMID: 24142684 [PubMed - as supplied by publisher]

Antithrombotic therapy in patients with atrial fibrillation and acute coronary syndrome in the real world: data from the Berlin AFibACS Registry.
CONCLUSIONS: These data suggest that in AFibACS patients, antithrombotic treatment focussed on dual antiplatelet therapy for ACS, rather than on anticoagulation therapy for stroke prevention. Factors influencing therapy at discharge were age, renal failure, stent implantation, AFib category, and CHA₂DS₂-VASc score. During the study period, triple therapy increased for stented patients. Optimal revascularization in diabetes after the FREEDOM trial: Were the controversies finally settled?
Abstract The prevalence of diabetes mellitus (DM) is growing worldwide. Prothrombotic and proinfl ammatory states, in adjunct to endothelial dysfunction and metabolic disorders, such as hyperglycemia, dyslipidemia, obesity, insulin resistance, and oxidative stress, are key features of the accelerated atherosclerotic progression observed in patients with DM. Moreover, drug-eluting stents (DES) thrombosis rate was higher in DM than in non-DM patients and DM itself was identifi ed as an independent predictor of stent thrombosis, particularly due to the impaired response to dual antiplatelet therapy. The accumulating ...

Predictors of mortality in patients with prosthetic valve infective endocarditis: A nation-wide multicenter study.
CONCLUSIONS: High creatinine level, poor functional status and high CRP measured on admission were independent risk associates for in-hospital mortality, whereas a NYHA class ofIII/IV and high CRP reflected independent risk for stroke/mortality end point. PMID: 23788308 [PubMed - in process]

Outcomes of invasive treatment in very elderly Polish patients with non-ST-segment-elevation myocardial infarction from 2003-2009 (from the PL-ACS registry).
CONCLUSIONS: Elderly patients with NSTEMI benefit significantly from invasive strategies and modern pharmacotherapy recommended by treatment guidelines. Nevertheless, this approach is associated with an increased incidence of major bleeding. PMID: 23558809 [PubMed - in process]

Influence of aerobic training on neurohormonal and hemodynamic responses to head-up tilt test and on autonomic nervous activity at rest and after exercise in patients after bypass surgery.
CONCLUSIONS: Aerobic training improved neurohormonal and hemodynamic responses to head-up tilt test and favorably modified sympatho-vagal balance in low risk post-CABG patients. PMID: 23558806 [PubMed - in process]

The Atrial Fibrillation in Turkey: Epidemiologic Registry AFTER.
This study aims to evaluate stroke risk in non-valvular atrial fibrillation (AF) and anticoagulant drug utilization within conformity to AF guidelines. AFTER (Atrial Fibrillation in Turkey: Epidemiologic Registry) is a prospective, multicentre study designed with the aim of describing the prevalence and epidemiology of AF practice in Turkey. Methods; Patients were recruited in 17 referral hospitals reflecting all the population of seven geographical regions of Turkey. 2242 consecutive patients who had been admitted with AF on their electrocardiogram were included in the study. 1745 of these patients, who had non-valvular AF...

The incidence and risk factors of stroke in patients with acute myocardial infarction treated invasively and concomitant impaired renal function.
Abstract BACKGROUND: Impaired : renal function is a marker of poor prognosis in patients with acute myocardial infarction (AMI). The aim of the study was to assess the incidence and independent predictors of stroke in population of patients with AMI treated invasively and concomitant impaired renal function (IRF). METHODS: We analyzed 2,520 consecutive AMI-patients admitted to our Center between 2003 and 2007 and treated with percutaneous coronary intervention. The whole population was divided into patients with IRF, defined as glomerular filtration rate < 60 ml/min/1.73m² or contrast induced nephropathy (IRF-...
