Established and potential echocardiographic markers of embolism and their therapeutic implications in patients with ischemic stroke.
Abstract Cardiogenic strokes comprised 11% of all strokes and 25% of ischemic strokes. An accurate identification of the cause of stroke is necessary in order to prepare an adequate preventive strategy. In this review the confirmed and potential causes of embolic strokes are presented, which can be detected in echocardiography in the context of present treatment guidelines and gaps in evidence. There remains a need for further studies assessing the meaning of potential cardiac sources of embolism and establishment of rules for optimal medical prevention (antiplatelet therapy [APT] vs. oral anticoagulation [OAC]) a...

Predictors of periprocedural complications in patients undergoing percutaneous coronary interventions within coronary bypass grafts.
CONCLUSIONS: Percutaneous coronary interventions of SVG is associated with increased risk of specific periprocedural complications. The ACS, slower TIMI flow before PCI and thrombectomy significantly increase the periprocedural complication rate in patients undergoing PCI of SVG. PMID: 29671862 [PubMed - as supplied by publisher]

The relationship between pulse waveform analysis indices, endothelial function and clinical outcomes in patients with peripheral artery disease treated using percutaneous transluminal angioplasty during a one-year follow-up period.
CONCLUSIONS: Endothelial function assessed as FMD and RH-PAT before PTA in patients with advanced PAD does not predict clinical outcomes during the one-year follow-up. < p > < /p >. PMID: 29611173 [PubMed - as supplied by publisher]

Sheep can be used as animal model of regional myocardial remodelling and controllable work.
CONCLUSIONS: Herein is developed a new sheep model with an asymmetrically remodelled LV. Simple pacemaker programming allows direct modulation of regional myocardial function and work. This model provides a new and valuable alternative for canine or porcine models and has the potential to become instrumental for investigating regional function and loading conditions on regional LV remodelling. Patent foramen ovale closure versus medical therapy after cryptogenic stroke: An updated meta-analysis of all randomized clinical trials.
CONCLUSION: Patent foramen ovale closure in adults with recent cryptogenic stroke was associated with a lower rate of recurrent strokes in comparison with medical therapy alone. PMID: 29512097 [PubMed - as supplied by publisher]

Renin-angiotensin system inhibition is associated with reduced risk of left atrial appendage thrombosis formation in patients with atrial fibrillation.
CONCLUSIONS: This study shows that RAS inhibitors may be effective in reducing the risk of LAAT in patients with AF through atrial reverse remodeling. PMID: 28980280 [PubMed - as supplied by publisher]

Left atrial appendage closure for primary primary prevention during percutaneous closure of septal defects in patients with large atria but no atrial fibrillation.
CONCLUSIONS: Concomitant closure of ASD or PFO in the presence of enlarged atria and LAA for primary primary prevention appears feasible and safe but has yet to prove its justification. PMID: 28840592 [PubMed - as supplied by publisher]

Percutaneous left atrial appendage occlusion: New perspectives for the method.
Abstract Ischemic stroke is a common complication of atrial fibrillation. Currently, oral anticoagulant drugs are the most commonly used method of stroke prevention. Left atrial appendage occlusion is thought to be the main source of thrombi in patients with AF. Percutaneous left atrial appendage is a valuable therapeutic option for selected high-risk patients with AF and contraindications for oral anticoagulation therapy. While complete closure of the left atrial appendage is the goal of a device implantation the variable nature of the left atrial appendage anatomy makes this goal difficult to achieve. Currently,...
