Ambulatory Hypertension Subtypes and 24-hour Systolic and Diastolic Blood Pressure as Distinct Outcome Predictors in 8341 Untreated People Recruited from 12 Populations.
CONCLUSIONS: -The risks conferred by DBP24 and SBP24 are age dependent. DBP24 and isolated diastolic hypertension drive coronary complications below age 50, whereas above age 50 SBP24 and isolated systolic and mixed hypertension are the predominant risk factors. Lack of Concordance between Empirical Scores and Physician Assessments of Stroke and Bleeding Risk in Atrial Fibrillation: Results from the Outcomes Registry for Better Informed Treatment of Atrial Fibrillation (ORBIT-AF) Registry.
CONCLUSIONS: There is little agreement between provider-assessed risk and empirical scores in AF. These differences may explain, in part, current divergence of anticoagulation treatment decisions from guideline recommendations. Temporal Relationship between Subclinical Atrial Fibrillation and Embolic Events.
CONCLUSIONS: Although SCAF is associated with an increased risk of stroke and embolism, very few patients had SCAF in the month prior to their event. CLINICAL TRIAL REGISTRATION: http://www.clinicaltrials.gov. Identifier: NCT00256152. PMID: 24633881 [PubMed - as supplied by publisher]

Platelet Activation after Radiofrequency Ablation in Atrial Fibrillation: Is There Any Clinical Implication?
Abstract Atrial fibrillation (AF) is the most common cardiac arrhythmia in the general population and in patients undergoing cardiac surgery(1). AF is a self-perpetuating arrhythmia which is facilitated by structural and functional changes elicited by atrial high rate activity. Shortening of atrial effective refractory period is the earliest functional change that characterizes atrial remodeling(2). As prevalence of AF increases with advancing age, its social impact is becoming very relevant because of the associated high risk of cardiovascular events and increased morbidity and mortality. AF is complicated by str...

The Impact of Partner Bereavement on Quality of Cardiovascular Disease Management.
CONCLUSIONS: Lower uptake of key cardiovascular care measures in the year before bereavement and reduced medication coverage after bereavement may contribute to increased cardiovascular risk. Clinicians need to ensure that quality of cardiovascular care is maintained in the pre and post bereavement period. PMID: 24255060 [PubMed - as supplied by publisher]

Plasminogen Activator Inhibitor-1: A Novel Therapeutic Target for Hypertension?
Abstract Hypertension is the most common reversible risk factor for stroke, myocardial infarction, and heart failure, affecting greater than 60 million individuals in the United State alone and 1 billion globally(1). According to the World Health Organization, hypertension is now the leading cause of preventable death worldwide(2). Importantly, the prevalence and complications of hypertension increase significantly with age. However, despite the availability of many anti-hypertensive agents of distinct pharmacologic classes and evidence-based guideline recommendations for step-wise, multi-drug regimens, control of...

Systemic Hypertension in Low Gradient Severe Aortic Stenosis with Preserved Ejection Fraction.
CONCLUSIONS: Systemic hypertension in LG severe AS with preserved EF is associated with elevated LV filling pressures and pulmonary hypertension. Treatment of hypertension with vasodilator therapy results in a lowering of the total LV afterload, with a decrease in LV filling pressures and pulmonary artery pressures. These findings have important implications for the management of patients with LG severe AS with preserved EF and hypertension. PMID: 23956211 [PubMed - as supplied by publisher]

Secular Trends in Cardiovascular Disease and its Risk Factors in Japanese: Half Century Data from the Hisayama Study (1961-2009).
CONCLUSIONS: Our findings suggest that in Japanese, the decreasing trends in the incidence of ischemic stroke have recently slowed down, and there has been no clear change in the incidence of acute myocardial infarction, probably because the benefits of hypertension control and smoking cessation have been negated by increasing metabolic risk factors. Elevated blood pressure in pregnancy and subsequent chronic disease risk.
CONCLUSIONS: Elevated blood pressure during pregnancy, regardless of type and even without known risk factors, signals high risk of later cardiovascular disease, chronic kidney disease, and diabetes mellitus. Clinical monitoring, risk factor evaluation, and early intervention could benefit women with hypertension in pregnancy. PMID: 23401113 [PubMed - in process]

Percutaneous Left Atrial Appendage Closure for Stroke Prophylaxis in Patients with Atrial Fibrillation: 2.3 Year Follow-Up of the PROTECT AF Trial.
CONCLUSIONS: The "local" strategy of LAA closure is noninferior to "systemic" anticoagulation with Warfarin. PROTECT AF has, for the first time, implicated the LAA in the pathogenesis of stroke in AF. CLINICAL TRIAL REGISTRATION INFORMATION: Clinicaltrials.gov; Unique Identifier: NCT00129545.
