Global Electrical Heterogeneity Risk Score for Prediction of Sudden Cardiac Death in the General Population: The Atherosclerosis Risk in Communities (ARIC) and Cardiovascular Health (CHS) Studies.
CONCLUSIONS: -Abnormal electrophysiological substrate quantified by GEH parameters is independently associated with SCD in the general population. Addition of GEH parameters to clinical characteristics improves SCD risk prediction. PMID: 27081116 [PubMed - as supplied by publisher]

Epidemiology and Mechanisms of Uremia-Related Cardiovascular Disease.
Abstract Patients with chronic kidney disease and end-stage renal disease are at 5- to 10-fold higher risk for developing cardiovascular disease (CVD) than age-matched controls. Clinically, CVD in this population manifests as coronary artery disease, arrhythmias, stroke, or congestive heart failure. Beyond the traditional risk factors (eg, diabetes mellitus and hypertension), uremia-specific factors that arise from accumulating toxins also contribute to the pathogenesis of CVD. In this review, we summarize the literature on the epidemiology of both traditional and uremia-related CVD and focus on postulated mechani...

SPS3 Evidence Supports Intensive Blood Pressure Control.
Abstract Interest in identifying the most appropriate targets for systolic blood pressure (SBP) lowering to reduce cardiovascular events in persons with hypertension has been piqued by the widely publicized results of the Systolic Blood Pressure Intervention Trial (SPRINT)(1,2). SPRINT found overwhelming benefit (25% reduction in the primary composite outcome of myocardial infarction (MI), acute coronary syndrome not resulting in MI, stroke, acute decompensated heart failure, or death from cardiovascular causes) and 27% reduction in all-cause mortality among participants randomized to a SBP target of < 120 mm H...

Understanding Population Cardiovascular Health: Harnessing the Power of Electronic Health Records.
Abstract In 2004 the American Heart Association (AHA) set forth an ambitious strategic goal for 2010 to 1) reduce death from coronary heart disease and stroke by 25%, 2) reduce the prevalence of smoking, high cholesterol, uncontrolled hypertension, and physical inactivity by 25%, and 3) to stop increases in obesity and diabetes relative to rates in 1999. Having achieved or exceeded goals 1 and 2 by 2008 through a number of collaborative efforts, including public education, practice guidelines and quality improvement initiatives, in 2009, the AHA reframed its focus more broadly to one of achieving increased levels ...

The Power of the Cluster.
Abstract The past 50 years have witnessed sea changes in cardiovascular medicine and research.(1) We came to recognize untreated hypertension as a highly malignant disorder and to recognize that medical treatment of hypertension leads to marked reductions in risk of stroke, myocardial infarction, and premature death. We also came to recognize the power of the randomized trial for assessing candidate strategies for prevention and treatment. Indeed, mega-trials conducted by cardiovascular clinician scientists have played a critical role in defining nearly every major evidence-based intervention, from hypertension tr...

Young Women With Acute Myocardial Infarction and the Post-Hospital Syndrome.
Abstract Our awareness and understanding of ischemic heart disease (IHD) in women has evolved substantially over the past two to three decades. The myth that heart attacks are "male diseases" was clearly dispelled and we know now that cardiovascular diseases (CVD), including heart disease and stroke, are common and represent the number one killer of women in the United States and worldwide.(1,2) In the Framingham Heart Study it was estimated that among women free of CVD at 50 years of age lifetime risk for developing CVD was 39.2%.(3) We have learned that clinically manifest IHD usually develops a decade later in ...

Community Pharmacy and Blood Pressure Control.
Abstract Widespread, effective identification and management of uncontrolled hypertension remains elusive, undermining our ability to reduce the impact of stroke and heart disease. The statistics are widely appreciated: roughly 30% of North Americans have hypertension and almost half are uncontrolled. Nearly 1,000 people a day die from complications attributed to hypertension, and direct and indirect costs amount to $46 billion a year in the United States alone.(1) Effective management of blood pressure for the 70 million adults in the US who currently have hypertension would clearly overwhelm our existing primary...

Risk of Intracranial Hemorrhage from Statin Use in Asians: A Nationwide Cohort Study.
CONCLUSIONS: -Generally, no association was observed between cumulative statin use and risk of ICH among subjects without a prior history of stroke. An increased risk was identified among the non-hypertensive cohort, but this finding should be interpreted with caution. PMID: 25858194 [PubMed - as supplied by publisher]

Stroke and Anticoagulation in Heart Failure Without Atrial Fibrillation: From Risk to Opportunity.
Abstract The association between heart failure and stroke risk has been described for more than three decades.(1) Hypercoagulability and left ventricular blood stasis are thought to contribute to stroke risk in patients with heart failure,(2) in addition to common risk factors such as hypertension and atrial fibrillation. Regardless of the mechanism, however, for the majority of patients with heart failure who do not have atrial fibrillation, this elevated stroke risk is appreciated but not managed. Assessing Neighborhood-Level Effects on Disparities in Cardiovascular Diseases.
Abstract Despite declines in mortality from cardiovascular diseases (CVD) and many CVD risk factors, CVD remains the leading cause of death in the US and racial and ethnic disparities persist. In 2010, rates of CVD mortality per 100,000 were: 192.2 for White women; 260.5 for Black women; 278.4 for White men; and 369.2 for Black men(1). In 2009-2010, metrics of ideal cardiovascular health factors (i.e., blood pressure, physical activity, healthy diet, healthy weight, smoking status, and glucose) were noted to be lower for Blacks and Mexican Americans than for Whites or other racial groups(1). In 2012, the following...
