In this non-prespecified, post hoc analysis of the AUGUSTUS (The Open-Label, 2 x 2 Factorial, Randomized, Controlled Clinical Trial to Evaluate the Safety of Apixaban vs. Vitamin K Antagonist and Aspirin vs. Placebo in Patients with Atrial Fibrillation and Acute Coronary Syndrome and/or Percutaneous Coronary Intervention) trial (Harskamp et al. J Am Coll Cardiol. 2022;79:417-27. PMID-35115097), the authors sought to evaluate whether the risk of bleeding or stroke as assessed by baseline HAS-BLED and CHA2DS2-VASc scores affected the results of the trial.

Accessory Pulmonary Vein Insertion into the Left Atrial Appendage Revealed During Intended Interventional LAA Occlusion
A 64-year-old male with paroxysmal atrial fibrillation and a recent stroke presented with internal carotid artery re-stenosis after prior angioplasty. Cardiovascular risk factors included arterial hypertension, hyperlipidemia, obesity, positive family history, heart failure, and active smoking. Due to high risk of cerebral ischemia as well as periinterventional bleeding complications, the patient was scheduled for interventional left atrial appendage (LAA) closure prior to carotid artery surgery.

Endocardial occlusion of incompletely surgically ligated left atrial appendage using an Amplatzer septal occluder device
A 55-year-old man underwent a surgical Maze procedure for the treatment of nonvalvular atrial fibrillation with concomitant surgical suture ligation of left atrial appendage (LAA). Baseline transesophageal echocardiography (TEE) and computed tomography (CT) angiography revealed a normal LAA free of thrombus (A). Ten days after surgery, he developed an acute embolic stroke.
