Editor's Choice – Risk of Stroke before Revascularisation in Patients with Symptomatic Carotid Stenosis: A Pooled Analysis of Randomised Controlled Trials
Current guidelines recommending rapid revascularisation of symptomatic carotid stenosis are largely based on data from clinical trials performed at a time when best medical therapy was potentially less effective than today. The risk of stroke and its predictors among patients with symptomatic carotid stenosis awaiting revascularisation in recent randomised controlled trials (RCTs) and in medical arms of earlier RCTs was assessed.

The current study aimed to further examine the possible impact of a confirmed SARS-CoV-2 infection on in hospital mortality.

Safety and Efficacy of Vacuum Assisted Thrombo-Aspiration in Patients with Acute Lower Limb Ischaemia: The INDIAN Trial
The aim was to evaluate the short term safety and effectiveness of the Penumbra/Indigo aspiration thrombectomy Systems (Penumbra Inc.) in patients with acute lower limb ischaemia. (ALLI). Recently, endovascular vacuum assisted thrombectomy devices, similar to those used in the management of acute ischaemic stroke, have become available for peripheral arteries, but data are still scarce.

Response to “Re Protamine and Myocardial Infarction Risk After Carotid Endarterectomy: A New Finding”
We would like to thank Francisco Marcos for his thoughtful insights on our recent study of protamine reversal of intra-operative heparin during carotid endarterectomy (CEA).1 As noted, our analysis demonstrated the irrefutable reduction in clinically significant bleeding events at the time of carotid surgery in patients treated with protamine. Moreover, this analysis determined that re-operation for bleeding events was associated with demonstrable increased rates of stroke, myocardial infarction (MI), and death.

Development and Internal Validation of a Risk Score to Detect Asymptomatic Carotid Stenosis
Asymptomatic carotid stenosis (ACS) is associated with an increased risk of ischaemic stroke and myocardial infarction. Risk scores have been developed to detect individuals at high risk of ACS, thereby enabling targeted screening, but previous external validation showed scope for refinement of prediction by adding additional predictors. The aim of this study was to develop a novel risk score in a large contemporary screened population.

Preventing Stroke Due to Intervention in the Aortic Arch
Stroke following endovascular intervention can be both clinically apparent and “silent”, detected incidentally on imaging. Silent brain infarcts (SBI) can be detected in up to 67%–80% patients undergoing thoracic aortic interventions.1,2 Why so many SBI do not lead to more apparent neurological deficits is a kind of miracle, as their distribution in the cerebral vascular bed is random. Their long term implications for cognitive decline are concerning but poorly defined, largely due to a lack of controlled studies with pre- and post-operative diffusion weighted MRI (DW-MRI), and medium to long term cognitive assessments.

Asymptomatic Carotid Stenosis: The Silent Threat
The study by Poorthuis et  al.1 reports on the development of the Prevalence of Asymptomatic Carotid Artery Stenosis (PACAS) risk score, which may reliably predict prevalence of asymptomatic carotid artery stenosis (ACAS) and identify individuals at high risk of stroke. The development of the risk score is well designed and based on 596 469 individuals attending population screening. This could become a useful tool in selecting individuals for targeted screening.

Chimney Endovascular Aneurysm Repair From Below is Attractive but Carries the Risk of a Relevant Complication
For the placement of chimney grafts, upper extremity access is necessary however, not all patients have suitable aortic arch anatomy, and it also carries a risk of stroke. In this context, successful chimney endovascular aneurysm repair (ChEVAR) via femoral access is attractive. Seven years ago, we presented the lift technique, which we used in 13 patients, placing self expanding covered stents approximately 1 –2 cm into the target renal artery from below. After fixation of the distal end with an angioplasty balloon, an 8 F sheath was carefully pushed over a stiff guidewire, lifting the chimney endograft upwards.

Comparison of 30 Day Stroke and Death in Hybrid Intervention and Open Surgical Reconstruction for the Treatment of Tandem Carotid Bifurcation and Supra-aortic Trunk Disease
The optimal approach for the treatment of tandem carotid bifurcation and supra-aortic trunk (SAT) disease remains controversial. The hybrid technique of carotid endarterectomy (CEA) with ipsilateral proximal endovascular intervention (IPE) has provided an attractive alternative to CEA with open SAT reconstruction (SATr). OCEANUS (PrOspective multiCentEr Study of cArotid Artery steNting Using mer Stent) Study: 30 Day and Two Year Results
Carotid artery stenting (CAS) with distal or proximal cerebral protection devices is an extensively developed method of endovascular treatment for atherosclerotic carotid stenosis, especially in high risk surgical candidates. Immediate and long term outcomes of CAS and carotid endarterectomy (CEA) have already been compared in CREST (Carotid Revascularization Endarterectomy vs. Stenting Trial), which included average risk patients. It should also be noted that CAS and CEA were only comparable regarding the composite end point; CEA proved to be superior to CAS regarding the incidence of ipsilateral stroke.
