Elevated EMMPRIN serum levels indicate plaque vulnerability in patients with asymptomatic high grade carotid stenosis
Carotid atherosclerosis, is an important cause of cerebral ischemic stroke. Sonographic plaques characteristics are inappropriate for exact prediction of possible future ischemic events. Additional markers are needed to forecast clinical outcome in high grade carotid stenosis. The aim of our study was to test extracellular matrix metalloproteinase inducer (EMMPRIN), due to the involvement in plaque formation/destabilization, as a potential marker of high-risk vulnerable plaques.

Multivariable Analysis of Pre-operative Brain Atrophy as a Predictor of Long Term Mortality After Carotid Endarterectomy
Brain atrophy is associated with an increased mortality rate in elderly trauma patients and in patients treated with mechanical thrombectomy for acute ischaemic stroke. In the setting of ischaemic stroke, the association between brain atrophy and death is stronger than that of sarcopenia. It has previously been shown that lower masseter area, as a marker of sarcopenia, is linked to lower survival after carotid endarterectomy (CEA). The aim of this study was to investigate whether brain atrophy is also associated with long term mortality in patients undergoing CEA.

In this issue, Abdallah et al1 provided alternative insights from a French Intestinal Stroke Centre, which adopts a conservative treatment algorithm for symptomatic IMAD.

Risk of bleeding complications per different perioperative antithrombotic regimes during carotid endarterectomy: a national registry analysis
Antithrombotic therapy is one of the cornerstones of the prevention of (recurrent) ocular or cerebral ischaemic events in patients with carotid artery stenosis. Randomized controlled trials on antithrombotic therapy for patients with minor ischaemic stroke and transient ischaemic attack (TIA) have recommended dual antiplatelet therapy (DAPT) in the three weeks following the index event. However, these trials excluded patients undergoing carotid revascularization. To date, the optimal antithrombotic therapy during the perioperative period of carotid endarterectomy (CEA) remains unclear.
