Response to the Comment on “Gait Impairments in Patients Without Lower Limb Hypertonia Early After Stroke Are Related to Weakness of Paretic Knee Flexors”
We thank the authors of the letter to the editor for reading our recent article1 and their comments regarding the reported relationships between gait impairments and the paretic knee flexors weakness in patients without lower limb muscle hypertonia early after stroke. The comments pertain to only one of the several study aims and the concerns reflect an oversimplification of the actual conclusions. In essence, the main concern is that clinicians may misinterpret our conclusions to suggest that the paretic knee flexors should be the sole or prime target of strengthening programs aimed at improving gait after stroke.

Can an Ankle Foot Orthoses with a Heel Lift Decrease Genu Recurvatum in Adults Post-Stroke?
Genu recurvatum (GR) in persons who are post-stroke can lead to excessive peak knee extension (>5 degrees) during the stance phase of gait. If untreated, GR can contribute to knee instability, weakness, osteoarthritis, and chronic pain. The purpose of this study was to investigate the effect of combining a heel lift with an ankle-foot orthosis (AFO) on genu recurvatum during gait in persons who are post-stroke. We hypothesized that the heel lift would result in a decrease in peak knee extension during the stance phase of gait.

Commentary: Associations Between Adherence to the Intervention Applied in the LAST Study (Life After Stroke) and Functional Outcome After Stroke
In this edition of the Archives Gunnes et  al present a post hoc analysis of a large study on outpatient adherence to a physical activity program for the poststroke patient.1,2 The results of this reanalysis of that 380 subject study found that increasing adherence to daily physical activity and weekly exercise was significantly associated , both clinically and statistically, with improved motor function, walking capacity, balance, and self-perceived functional outcomes after stroke.1

The role of self-efficacy in the predictive relationship of motor ability to functional performance after task-related training in stroke: A secondary analysis of longitudinal data
Objective: To determine whether self-efficacy (SE) mediates or moderates the relationship between motor ability at pretest and functional use of the affected arm at posttest in task-related training for stroke.Design: Retrospective, observational cohort study.Setting: Outpatient rehabilitation settings.Participants: Eighty patients with chronic stroke.Interventions: The training was delivered to the participants for 60 to 90 minutes/session, 3 to 5 sessions/week for 4 to 6 weeks. The training involved specific robot-assisted, mirror, or combined therapy, followed by functional task practice for approximately 30 minutes in each session.
