Regional Practice Patterns and Outcomes of Surgery for Acute Type A Aortic Dissection.
CONCLUSIONS: Operative morbidity and mortality remain significant for type A aortic dissection, but lower than historical outcomes. The extent of aortic surgery has increased, resulting in adaptive cerebral protection changes in contemporary "real-world" practice. PMID: 28599962 [PubMed - indexed for MEDLINE]

Florida Sleeve Procedure Is Durable and Improves Aortic Valve Function in Marfan Syndrome Patients.
CONCLUSIONS: The FS procedure can be performed safely in Marfan syndrome patients with immediate improvement in aortic valve function. Long-term survival and freedom from reoperation rates are encouraging. PMID: 28410640 [PubMed - indexed for MEDLINE]

The Impact of Thoracic Endovascular Aortic Repair on Long-Term Survival in Type B Aortic  Dissection.
CONCLUSIONS: The treatment of uncomplicated aTBAD with optimal medical therapy results in a high incidence of surgical intervention and poor long-term survival. At the index hospitalization, TEVAR may confer a survival advantage and serve as optimal therapy for complicated and uncomplicated aTBAD patients. PMID: 28811003 [PubMed - as supplied by publisher]

Hypothermia and Selective Antegrade Cerebral Perfusion Is Safe for Arch Repair in Type A Dissection.
CONCLUSIONS: Unilateral selective antegrade cerebral perfusion with moderate hypothermic circulatory arrest remains a safe strategy for cerebral protection during emergent surgical repair of acute type A dissection and provides equivalent outcomes for both limited and extensive aortic arch reconstruction. Based on these data, unilateral selective antegrade cerebral perfusion and moderate hypothermic circulatory arrest may represent an optimal strategy for cerebral protection in this acute setting. Outcomes of Antegrade Stent Graft Deployment During Hybrid Aortic Arch Repair.
CONCLUSIONS: Hybrid repair of complex aortic arch pathology with antegrade stent graft deployment can be performed safely with high technical success while obviating the need for a second operation. Reasonable midterm survival can be anticipated; however, older age, preoperative coagulopathy, and postoperative acute kidney injury are factors associated with poor outcome. Random Finding of a Ruptured Kommerell Aneurysm After  Stroke.
We report the case of a 74-year-old man who received an incidental diagnosis of a contained rKA involving an aberrant right subclavian artery after a stroke. He underwent successful hybrid repair with the use of thoracic endovascular aortic repair associated with cervical debranching of the left subclavian artery and vascular plug occlusion of both subclavian artery origins. Whereas no early adverse event was reported, the patient experienced a vertebrobasilar insufficiency once he had recovered, requiring an additional right carotid-to-subclavian bypass. PMID: 28007277 [PubMed - in process]

Aortic Remodeling After Endovascular Repair of Complicated Acute Type B Aortic  Dissection.
CONCLUSIONS: TEVAR for complicated aTBAD results in low 30-day and 1-year mortality rates, with higher reintervention rates than observed with open operations. TEVAR is effective in thrombosing and stabilizing the size of the thoracic FL. The abdominal aortic FL remains patent and must be carefully scrutinized for long-term aneurysm formation. Open Repair of Thoracoabdominal Aortic Aneurysm in Patients 50 Years Old and  Younger.
CONCLUSIONS: Early outcomes of open TAAA repair were excellent for patients 50 years old or younger, despite the fact that these patients typically required extensive repairs. PMID: 27938888 [PubMed - as supplied by publisher]

The Society of Thoracic Surgeons Mitral Valve Repair/Replacement Plus Coronary Artery Bypass Grafting Composite Score: A Report of The Society of Thoracic Surgeons Quality Measurement Task Force.
CONCLUSIONS: An STS composite performance measure was developed for MVRR + CABG operations. This measure may be useful for outcome assessment, quality improvement, patient counseling, clinical research, and public reporting. PMID: 27938886 [PubMed - as supplied by publisher]

Scimitar Syndrome Repair in Adults: Intermediate-Term Results Using an Extracardiac Conduit.
CONCLUSIONS: Correction of scimitar syndrome with an extracardiac conduit can be easily and safely performed in adults, with excellent intermediate-term durability, without graft thrombi or stenosis. This technique avoids deep hypothermic circulatory arrest when the SV is short or enters the IVC in an unusually caudad location. PMID: 27424466 [PubMed - as supplied by publisher]

Branched Endovascular Therapy of the Distal Aortic Arch: Preliminary Results of the Feasibility Multicenter Trial of the Gore Thoracic Branch Endoprosthesis.
CONCLUSIONS: Total endovascular repair of distal zone 2 arch aortic aneurysms can be achieved with a novel branched arch endograft. Future studies will evaluate the feasibility of this approach for aneurysms encompassing the brachiocephalic trunk and left carotid artery. PMID: 27262912 [PubMed - as supplied by publisher]

Bicuspid Aortic Insufficiency With Aortic Root Aneurysm: Root Reimplantation Versus Bentall Root Replacement.
CONCLUSIONS: In patients with bicuspid aortic valve AI with root aneurysm, primary cusp repair with root reimplantation achieves equivalent midterm outcomes compared with Bentall root replacement. PMID: 27261086 [PubMed - as supplied by publisher]

Results of Open Surgical Repair in Patients With Marfan Syndrome and Distal Aortic Dissection.
CONCLUSIONS: In these patients with MFS with aortic dissection, open TAAA repair incurred reasonable operative risk, but improvements are needed to reduce rates of renal failure. Extent II TAAA repair does not appear to increase operative risk in patients with MFS. PMID: 26876340 [PubMed - as supplied by publisher]

Moderate Ischemic Mitral Regurgitation After Posterolateral Myocardial Infarction in Sheep Alters Left Ventricular Shear but Not Normal Strain in the Infarct and Infarct Borderzone.
CONCLUSIONS: Moderate CIMR alters radial circumferential shear strain after posterolateral MI in sheep. Further studies are needed to determine the effect of shear strain on myocyte hypertrophy and the effect of mitral repair on myocardial strain. PMID: 26857634 [PubMed - as supplied by publisher]

Left Subclavian Arterial Coverage and Stroke During Thoracic Aortic Endografting: A Systematic Review.
We report a systematic review of 63 studies comprising more than 3,000 patients. We conclude that stroke risk after TEVAR is increased by LSA coverage, and that LSA revascularization reduces stroke risk. LSA revascularization may lower the rate of posterior stroke. TEVAR for aneurysm is associated with increased stroke risk compared to TEVAR for dissection. PMID: 26588864 [PubMed - as supplied by publisher]
