Assessment of the relationship between platelet reactivity, vascular risk factors and gender in cerebral ischaemia patients.
CONCLUSIONS: There are no differences between platelet reactivity and the incidence of aspirin resistance in patients with stroke and TIA. Male gender and smoking are associated with greater reactivity of platelets and more frequent occurrence of acetylsalicylic acid resistance in patients with cerebral ischaemia. CLINICAL IMPLICATIONS: Dual antiplatelet therapy or clopidogrel treatment should be considered in smoking males with cerebral ischaemia due to the high risk of aspirin inefficiency. Statement by a Working Group conceived by the Polish National Consultants in Cardiology and Neurology addressing the use of implantable cardiac monitors in patients after ischaemic embolic stroke of undetermined source.
Abstract INTRODUCTION: Stroke remains one of the main causes of death and the most common cause of long-term disability in adults. Embolic strokes of undetermined source (ESUS) amount to a significant proportion of all ischaemic strokes. Detection of atrial fibrillation (AF) in this group of patients would allow for a major therapeutic decision to switch from antiplatelets to oral anticoagulants and therefore significantly reduce the risk of recurrence. STATE OF THE ART: Current technology allows long-term continuous ECG monitoring with different systems, including implantable cardiac monitors (ICM). However,...

Assessment of relationship between C-reactive protein to albumin ratio and 90-day mortality in patients with acute ischaemic stroke.
CONCLUSIONS AND CLINICAL IMPLICATIONS: Our study demonstrated that a high CAR value is an independent predictor of 90-day mortality in patients with AIS. PMID: 31145464 [PubMed - as supplied by publisher]

Transthoracic echocardiography in the assessment of cardiogenic causes of ischaemic stroke.
CONCLUSIONS: 1. Transthoracic echocardiography performed routinely in all AIS patients affects the treatment in a very low percentage of cases. 2. The group that could benefit the most from TTE examination includes people without established indications for chronic anticoagulant therapy, in particular patients after myocardial infarction and people with additional clinical symptoms. 3. In patients with AIS, the diagnostic sensitivity of TTE in the detection of PFO is low. Young people with a cryptogenic ischaemic stroke should undergo a transoesophageal assessment. Analysis of echocardiographic parameters of cardiac function in patients with acute stroke.
CONCLUSIONS: One quarter of patients in the acute phase of stroke have LVSD and/or LVDD. LVSD adversely affects both neurological status in acute stroke as well as functional status in the short-term follow-up. Age at first-in-life stroke incidence and lower EF are predictors of poor functional status one month after a stroke. PMID: 30855702 [PubMed - as supplied by publisher]

Stroke and TIA mimics in patients referred to a neurological emergency department by non-ambulance physicians, ambulance physicians and paramedics.
CONCLUSIONS: There seem to be no major differences in the structure, early diagnostic approach and management of ACE mimics between referrals from ambulance physicians and ambulance paramedics, which provides reassurance to the healthcare systems that rely solely on paramedics. Mimics referred by non-ambulance physicians appear different in structure and less resource consuming. PMID: 30614515 [PubMed - as supplied by publisher]

Age as a prognostic factor in relation to surgical evacuation of spontaneous supratentorial intracerebral haemorrhage.
CONCLUSIONS AND CLINICAL IMPLICATIONS: Neurosurgical treatment of the sICH patients is indicated only if age and potentially improved morbidity is carefully evaluated considering the STICH and this study; otherwise, we will just increase the health care burden with a number of extremely care-dependent patients. Cerebral thrombolysis in patients with ischemic stroke and heart failure.
CONCLUSION: The presence of systolic HF, defined according to an old and a new definition, does not determine safety and efficacy of cerebral iv-thrombolysis in patients with IS. PMID: 30170703 [PubMed - as supplied by publisher]

sP- and sE-selectin in stroke patients with metabolic disorders.
CONCLUSION: Soluble E- and P-selectin, blood markers of vascular injury and platelet activation, could be useful in the assessment of atherothrombotic properties of hyperglycemia and hyperlipidemia in stroke patients. PMID: 30190210 [PubMed - as supplied by publisher]

MMP-9 and/or TIMP as predictors of ischaemic stroke in patients with symptomatic and asymptomatic atherosclerotic stenosis of carotid artery treated by stenting or endarterectomy - A review.
Abstract We still lack an optimal tool to predict ischaemic stroke in patients with symptomatic and asymptomatic carotid stenosis (CS). It has already been shown that patients at increased risk of ischaemic stroke can be identified based on the elevated plasma levels of metalloproteinases (MMPs) and reduced activity tissue inhibitor of metalloproteinase (TIMP). There are few studies presenting the role of MMP-9 and TIMP in ischaemic stroke both in patients with symptomatic and asymptomatic CS treated with stenting or endarterectomy, however we have not found any published review summarizing the role of abovementio...

Future challenges of stroke treatment.
Abstract Posterior circulation stroke accounts for approximately 20% of all ischaemic strokes. Acute basilar artery occlusion (BAO) is one of the most severe conditions, it is associated with death or major disability in more than three quarters of the cases, and its optimal management remains unestablished. Currently, the treatment is based primarily upon consensus, the clinical practice varies widely, and the actual benefit of mechanical thrombectomy has to be fully estimated. Although the recent years have profoundly revolutionized and improved the stroke care, many questions still remain unanswered and will re...

Looking for novel, brain-derived, peripheral biomarkers of neurological disorders.
Abstract The role of blood brain barrier (BBB) is to preserve a precisely regulated environment for proper neuronal signaling. In many of the central nervous system (CNS) pathologies, the function of BBB is altered. Thus, there is a necessity to evaluate a fast, noninvasive and reliable method for monitoring of BBB condition. It seems that revealing the peripheral diagnostic biomarker whose release pattern (concentration, dynamics) will be correlated with clinical symptoms of neurological disorders offers significant hope. It could help with faster diagnosis and efficient treatment monitoring. In this review we su...

Cerebral vasomotor reactivity predicts the development of acute stroke in patients with internal carotid artery stenosis.
CONCLUSION: Decreased VMR value is an important prognostic factor for the occurrence of aortic atherosclerotic stroke, and can be used as a reference for preoperative hemodynamic evaluation in patients with internal carotid artery stenosis. PMID: 29361283 [PubMed - as supplied by publisher]

Free thyroxine and TSH interact with secreted protein acidic and rich in cysteine-like 1 in ischemic stroke.
Abstract The role of the thyroid gland in ischemic stroke pathology is not well understood. As thyroid hormones modulate the extracellular matrix, we explored the possible link between them and secreted protein acidic and rich in cysteine like 1 (SC1) - one of the extracellular matrix molecules. In the 81 patients with acute ischemic stroke, serum SC1 levels were much higher compared with 30 control subjects: 4.47 vs 2.43ng/mL (p<0.001). Serum levels of free thyroxine (fT4) were higher in stroke subjects compared to those of controls (p=0.03). In stroke patients, TSH concentration was lower than in the control ...
