Does compensatory hyperparathyroidism predispose to ischemic stroke? Decreased bone mass and increased bone turnover with valproate therapy in adults with epilepsy; An alternative to vitamin D supplementation to prevent fractures in patients with MS; High prevalence of vitamin D deficiency and reduced bone mass in Parkinson's disease
Because of known issues leading to several retractions of papers by Y. Sato et al., the editors of Neurology® have, as a matter of due diligence, researched other papers that this group published in Neurology. To ensure that the scientific literature is correct, the Editor chooses to publish an Expression of Concern regarding 3 observational studies and a Letter to the Editor published in Neurology prior to the retracted clinical trials.1–4

Direct oral anticoagulant- vs vitamin K antagonist-related nontraumatic intracerebral hemorrhage
Conclusions: DOAC-related ICH is associated with smaller baseline hematoma volume and lesser neurologic deficit at hospital admission compared to VKA-related ICH.

Long-term antithrombotic treatment in intracranial hemorrhage survivors with atrial fibrillation
Conclusions: In observational studies, anticoagulation with VKA is associated with a lower rate of IS than APA or no-ATM without increasing ICH recurrence significantly. A randomized controlled trial is needed to determine the net clinical benefit of anticoagulation in ICH survivors with AF.

Letter re: Early start of DOAC after ischemic stroke: Risk of intracranial hemorrhage and recurrent events
Seiffge et al.1 evaluated the effect of direct, non–vitamin K antagonist oral anticoagulants (DOACs) or vitamin K antagonists on intracranial hemorrhage (ICH) and recurrent ischemic events in patients with acute ischemic stroke or TIA and nonvalvular atrial fibrillation (NVAF). I have a concern about the study.

Author response: Early start of DOAC after ischemic stroke: Risk of intracranial hemorrhage and recurrent events
We thank Prof. Kawada for his interest in our article.1 We agree that larger patient cohorts more accurately estimate the risk of intracranial hemorrhage (ICH) from direct, non–vitamin K antagonist oral anticoagulants (DOACs). However, the definition of ICH is crucial. In our study we looked at symptomatic ICH (sICH) only, and observed only 1 patient with sICH.1 In contrast, the cited study of Cappellari et al.2 looked at both sICH and asymptomatic ICH: Of the 7 patients with any ICH after DOAC start, only 1 had sICH. Therefore, the rates of sICH were comparable between the 2 studies.

Meta-analysis of folic acid efficacy trials in stroke prevention: Insight into effect modifiers
Conclusions: Folic acid supplementation could reduce the stroke risk in regions without folic acid fortification, particularly in trials using a relatively low dosage of folic acid and with low vitamin B12 levels.

Safety of OnabotulinumtoxinA Treatment With Concomitant Antithrombotic Therapy in Patients With Post-stroke Spasticity: A Pooled Analysis of Randomized, Double-Blind Studies (S56.008)
Conclusions:There is no apparent increased risk of bleeding complications in patients on antithrombotics following IM onabotA treatment; nonetheless, careful observation of the injection site and patient education of the potential for bleeding complications remain warranted.Study Supported by: Allergan plc, Dublin, IrelandDisclosure: Dr. Dimitrova has received personal compensation for activities with Allergan as an employee. Early start of DOAC after ischemic stroke: Risk of intracranial hemorrhage and recurrent events
Conclusions: Even if DOACs are often started early after an index event, the risk of ICH appears to be low. Among all patients receiving anticoagulation, the rate of recurrent events was 6 times higher than the rate of ICH.

Safety of Mechanical Thrombectomy for Acute Ischemic Stroke in Patients on Novel Oral Anticoagulants (P1.196)
Conclusion: Patients on NOAC undergoing mechanical thrombectomy show a trend towards increased sICH compared to the warfarin group. This may warrant larger studies of safety of mechanical thrombectomy in the presence of NOACs.Disclosure: Dr. Sharma has nothing to disclose. Dr. Atchaneeyasakul has nothing to disclose. Shared genetic susceptibility of vascular-related biomarkers with ischemic and recurrent stroke
Conclusions: Our data identify a genetic contribution to inflammatory and hemostatic biomarkers in a stroke population. Additionally, our results suggest shared genetic contributions to circulating CRP levels measured poststroke and risk for incident and recurrent ischemic stroke. These data broaden our understanding of genetic contributors to biomarker variation and ischemic stroke risk, which should be useful in clinical risk evaluation.
