Researchers get a handle on how to control blood sugar after stroke
(NIH/National Institute of Neurological Disorders and Stroke) Hyperglycemia, or high levels of glucose, is common in patients with acute ischemic stroke and is associated with worse outcomes compared to normal blood sugar levels. Animal studies also pointed to an effect of high blood sugar in worsening stroke injury. Stroke experts have debated whether intensive glucose management after acute ischemic stroke leads to better outcomes but a new study in JAMA finds that aggressive methods are not better than standard approaches.

Antioxidant treatment in acute ischemic stroke may delay the onset of Alzheimer's dementia
(Bentham Science Publishers) There is a considerable overlap between vascular risk factors and risk factors of Alzheimer's disease (AD). In addition, incident stroke approximately doubles the risk of developing AD. Oxidative stress is significantly involved in the pathogenesis of AD and suffers a dramatic increase in the setting of acute ischemic stroke, especially in cardioembolic stroke, followed by lacunar stroke. Dampening this oxidative burst could delay the onset of subsequent dementia in stroke survivors.

Risk of second stroke can be reduced with prevention efforts based on cause of first stroke
(American Heart Association) Having a stroke caused by blocked blood vessels or a transient ischemic attack (TIA) greatly increases your chances of having a future stroke. Identifying the cause or causes of the first stroke is key to developing strategies to prevent additional strokes.Managing blood pressure levels, reducing or quitting smoking, eating a healthy diet, and regular physical activity will reduce the risk of a second stroke, along with managing conditions such as Type 2 diabetes and high cholesterol.

Stroke patients past the 90-day danger period remain at high risk for repeat event

International study suggests combination therapy may prevent stroke in certain people
(NIH/National Institute of Neurological Disorders and Stroke) Results from an international clinical trial of more than 4880 participants, published in the New England Journal of Medicine, show that combining clopidogrel and aspirin following a small stroke or experiencing minor stroke symptoms decreases risk of a new stroke, heart attack or other ischemic event within 90 days. The combination therapy was also associated with an increase in major bleeding, although many of those episodes were non-fatal and did not occur in the brain.

Nurse work environment influences stroke outcomes
(University of Pennsylvania School of Nursing) Stroke remains a leading cause of death worldwide and one of the most common reasons for disability. While a wide variety of factors influence stroke outcomes, data show that avoiding readmissions and long lengths of stay among ischemic stroke patients has benefits for patients and health care systems alike. Although reduced readmission rates among various medical patients have been associated with better nurse work environments, it is unknown how the work environment might influence readmissions and length of stay for ischemic stroke patients.

Study supports intracerebral stem cell injections to prevent/reduce post-stroke cognitive deficits
(IOS Press) Cognitive deficits following ischemic stroke are common and debilitating, even in the relatively few patients who are treated expeditiously so that clots are removed or dissolved rapidly and cerebral blood flow restored. A new study in Restorative Neurology and Neuroscience demonstrates that intracerebral injection of bone-marrow-derived mesenchymal stem cells (BSCs) reduces cognitive deficits produced by temporary occlusion of cerebral blood vessels in a rat model of stroke, suggesting that BSCs may offer a new approach for reducing post-stroke cognitive dysfunction.

Substance naturally found in humans is effective in fighting brain damage from stroke
(Henry Ford Health System) A molecular substance that occurs naturally in humans and rats was found to 'substantially reduce' brain damage after an acute stroke and contribute to a better recovery, according to a newly released animal study by researchers at Henry Ford Hospital.The study, published online before print in Stroke, the journal of the American Heart Association, was the first ever to show that the peptide AcSDKP provides neurological protection when administered one to four hours after the onset of an ischemic stroke.

Incidence of stroke in the elderly has dropped by 40 percent over the last 20 years
(Elsevier Health Sciences) A new analysis of data from 1988-2008 has revealed a 40 percent decrease in the incidence of stroke in Medicare patients 65 years of age and older. This decline is greater than anticipated considering this population's risk factors for stroke, and applies to both ischemic and hemorrhagic strokes. Investigators also found death resulting from stroke declined during the same period. Their findings are published in the July issue of The American Journal of Medicine.

Medical costs for stroke survivors stay high 10 years on
(Monash University) New data shows that healthcare and personal costs to support survivors of stroke remains high 10 years on. The Monash University research, published today in the journal Stroke, is the first to look at the long-term costs for the two main causes of stroke; ischemic where the blood supply stops due to a blood clot, and hemorrhagic, which occurs when a weakened blood vessel supplying the brain bursts.

USF researchers find stroke damages blood-spinal cord barrier
(University of South Florida (USF Health)) Researchers investigating the short and long-term effects of ischemic stroke in a rodent model have found that stroke can cause long-term damage to the blood-spinal cord barrier, which provides a specialized protective 'microenvironment' for neural cells in the spinal cord, creating a 'toxic environment' in the spinal cord that might leave stroke survivors susceptible to motor dysfunction and disease pathology.

First systemic evidence for safety of tPA in stroke patients with sickle cell disease
(Medical University of South Carolina) Adult patients with sickle cell disease who experience a stroke caused by a clot (i.e., ischemic strokes) can be treated safely with tissue plasminogen activator if they qualify, report investigators at the Medical University of South Carolina and elsewhere in the March 2017 issue of Stroke. The investigators analyzed in-hospital patient data obtained from the Get With The Guidelines -- Stroke database.

Brain-scan guided emergency stroke treatment can save more lives
(NIH/National Institute of Neurological Disorders and Stroke) Advances in brain imaging can identify a greater number of stroke patients who can receive therapy later than previously believed, according to a new study. The results of the Endovascular Therapy Following Imaging Evaluation for the Ischemic Stroke (DEFUSE 3) trial demonstrated that physically removing brain clots up to 16 hours after symptom onset in selected patients led to improved outcomes compared to standard medical therapy. The study was funded by the National Institutes of Health.

Stroke prevention drug combo shows promise, study says
(University of Texas at Austin) If you've had a minor stroke or a transient ischemic stroke (TIA), taking the clot-preventing drug clopidogrel along with aspirin may lower your risk of having a major stroke within the next 90 days, according to new research published in The New England Journal of Medicine.
