A case of Lemierre's syndrome causing panhypopituitarism, cavernous sinus thrombosis, ischaemic stroke and pulmonary embolism
We present the case of a previously well 19-year-old man, who presented acutely unwell with meningitis and sepsis. Fusobacterium necrophorum was isolated from peripheral blood cultures and identified on cerebrospinal fluid with 16S rDNA Polymerase Chain Reaction (PCR). Imaging demonstrated internal jugular vein thrombosis with subsequent cavernous venous sinus thrombosis. Pituitary function tests were suggestive of panhypopituitarism. The patient was diagnosed with Lemierre's syndrome complicated by meningitis, cavernous sinus thrombosis, base of skull osteomyelitis, ischaemic stroke and panhypopituitarism. He was treated ...

J R Coll Physicians Edinb. 2023 Jul 25:14782715231187440. doi: 10.1177/14782715231187440. Mainly genetic disorders, family history, hypertension and smoking put patients at high risk of intracranial aneurysms formation. MR angiogram, CT angiogram or conventional angiography can detect aneurysms 5 mm or larger in size. Fusiform aneurysms are rare and mainly due to dissection or atherosclerosis. Majority of these are in the posterior circulation and represent 3-13% of all intracranial aneurysms. T...

Platypnoea orthodeoxia syndrome and its association with cryptogenic ischaemic stroke
J R Coll Physicians Edinb. 2023 Mar 4:14782715231159470. doi: 10.1177/14782715231159470. Online ahead of print.ABSTRACTPlatypnoea orthodeoxia syndrome is a rare condition, which can be present in patients with patent foramen ovale (PFO). In this case, a 72-year-old lady presented to the emergency department with a cryptogenic stroke associated with right thalamic infarct. While in hospital, it was noticed that the patient had desaturations in the upright position, and these improved in the recumbent position, consistent with platypnoea orthodeoxia syndrome. The patient was found to have a PFO, which was then closed, and he...
