E-190 Trends in admissions for intracranial dissections in the United States
ConclusionThe prevalence of uIADs among hospitalized patients is scarce, and only 2% of craniocervical dissection-related AIS is due to uIAD. Compared to IAs, patients were more likely younger and male, and uIAD more commonly led to acute ischemic stroke and motor deficits. The trend in age remained stable across the four years analyzed, while the proportion of females increased. There was no trend in the proportion of races among uIADs, however.Disclosures W. Mechanical thrombectomy beyond the circle of Willis: efficacy and safety of different techniques for M2 occlusions
Conclusion Stent-retrievers and a combined approach for M2 occlusions seem more effective than direct aspiration, but with higher rates of iSAH. This leads to no detectable difference in clinical outcome at 3 months.

Revascularization of Hemorrhagic Moyamoya Disease in a North American Cohort: The Role of Timing in Perioperative and Long-Term Outcomes
BACKGROUND: Several East Asian studies have examined the role of revascularization in the context of hemorrhagic moyamoya disease (MMD) and found a decrease in postoperative rehemorrhage rates. To date, no data exist comparing revascularization with conservative management in North American patients with hemorrhagic MMD. OBJECTIVE: To compare the clinical outcomes of conservative management vs surgical revascularization for North American patients with hemorrhagic MMD and investigate the effect of revascularization timing. METHODS: We retrospectively studied the mortality and stroke-free survival of patie...

Rates of adverse events in patients with ischemic stroke treated at thrombectomy capable hospitals
Conclusions A decrease in-hospital adverse events and improved discharge outcomes were observed among patients with ischemic stroke admitted to a TCH compared with a non-TCH.

Comparative study of on-label versus off-label treatment of intracranial aneurysms with the Pipeline embolization device
CONCLUSIONS: In real-world practice, off-label use of PED is common and can achieve similar efficacy as on-label use. However, in aggregate, off-label use was found to carry an increased rate of ischemic complications. With judicious attention to safety and individual patient characteristics, these results highlight the scale and general feasibility of off-label PED use by experts.PMID:35090133 | DOI:10.3171/2021.11.JNS212481

Periprocedural safety of saccular aneurysm embolization with the Penumbra SMART Coil System: a SMART registry subset analysis
Conclusion The low rates of thromboembolic complications and RRP events demonstrate the adequate safety profile of the SMART Coil System to treat cerebral aneurysms in routine clinical practice. Trial registration number NCT02729740.

Duration of symptomatic stroke and successful reperfusion with endovascular thrombectomy for anterior circulation large vessel occlusive stroke
Conclusion In a real-world cohort of acute ischemic stroke patients with anterior circulation occlusion treated with endovascular therapy, longer duration of symptomatic stroke is associated with lower rates of successful reperfusion and reperfusion on the first pass.

Correlation between intracranial vertebral artery stenosis diameter measured by digital subtraction angiography and cross-sectional area measured by optical coherence tomography
Conclusion Measurement of the narrowest diameter was not a reliable predictor of the cross-sectional area of V4 stenosis. Larger studies are therefore needed to develop a new evaluation system based on V4 stenosis.

COVID-19 meets neurointervention on the pages of JNIS
As 2019 came to a close, a novel B-coronavirus, labeled by the World Health Organization as COVID-19, was noted to be spreading in China.1 The COVID-19 virus is an enveloped single-strand, positive-sense RNA, which means that the virus can use its RNA as the template from which to create proteins needed for propagation and, ultimately, spread. The name coronavirus refers to the characteristic crown-like appearance of these viruses that is seen on electron microscopy. By spring 2020, COVID-19 was spreading rapidly throughout the United States and Europe, and neurointerventionalists soon found themselves on the frontline.2 T...

O-005 Comparison of on-label versus off-label treatment of intracranial aneurysms with the pipeline embolization devices
ConclusionIn real-world practice, off-label uses of PED can achieve similar safety and efficacy to on-label uses, though there may be a slightly higher rate of ischemic complications in off-label uses. Expert judgment is a useful supplement to official guidelines when assessing reasonable PED use beyond its approved indications.Disclosures S. Cler: None. D. Lauzier: None. A. O-010 Racial disparity in mechanical thrombectomy utilization: multicenter registry results from 2016-2020
ConclusionsBlack stroke patients underwent MT less frequently than Whites, likely in part due to longer times from LKW to hospital arrival and a lower rate of documented aLVO. Further studies are needed to assess whether extending the MT time window and more aggressive LVO screening protocols mitigate this disparity.Disclosures A. Wallace: None. D.
