O-016 Aspiration thrombectomy for acute ischemic stroke patients presenting after 6 hours
ConclusionFor patients with late presentation stroke, aspiration thrombectomy with the Penumbra System in anterior circulations is safe and effective. Rates of good functional outcome and all-cause mortality compare favorably to the DAWN and DEFUSE-3 medical management arms.Disclosures A. Mechanical thrombectomy beyond the circle of Willis: efficacy and safety of different techniques for M2 occlusions
Conclusion Stent-retrievers and a combined approach for M2 occlusions seem more effective than direct aspiration, but with higher rates of iSAH. This leads to no detectable difference in clinical outcome at 3 months.

E-123 Evaluation of safety and efficacy of transradial access for mechanical thrombectomy inacute ischemic stroke - A single center experience
ConclusionTransradial access for mechanical thrombectomy in acute ischemic stroke is a primary access in tortuous vasculature/posterior circulation strokes and is a good alternative to the femoral approach. This vascular access is especially helpful in patients with posterior circulation strokes, on anticoagulation or with large body habitus.Disclosures A. Kuhn: None. J. Singh: None. E-040 Factors associated with failed mechanical thrombectomy in acute ischemic stroke
ConclusionFailed MT was encountered in approximately 12% of cases. The most common location of failed MT was intracranial and this was predominantly due to ICAS. Further studies to evaluate better treatment modalities of ICAS related LVO are warranted.Disclosures O. Lajthia: None. H. Ali: None. R. Neyes: None. E. Almallouhi: None. R. Chalhoub: None. K. E-147 Ephrin receptor beta 1 single nucleotide polymorphisms in association with age in acute stroke patients undergoing thrombectomy
Conclusion193 patients were included in this interim analysis across 23 centers in the United States, between 02/2021 and 11/2021. For the EPHB1 SNP (rs13073658), Table 1 shows the numbers and proportions of patients with varying expressions of the SNP. While no patients 50 or older were homozygous for the SNP, 16% of those under 50 years old were. The potential relationship between EPHB1 SNP and stroke in younger patients requires further evaluation.Abstract E-147 Table 1Disclosures J. E-111 Repeat thrombectomy after large vessel occlusion stroke: incidence, clinical and technical outcomes

Economic evaluation of intravenous alteplase for stroke with the time of onset between 4.5 and 9 hours
Conclusion For patients with AIS with a time of onset between 4.5 and 9 hours, perfusion imaging-guided intravenous alteplase was likely to be cost-effective in China and was cost-effective in the US when compared with placebo.

O-010 Racial disparity in mechanical thrombectomy utilization: multicenter registry results from 2016-2020
ConclusionsBlack stroke patients underwent MT less frequently than Whites, likely in part due to longer times from LKW to hospital arrival and a lower rate of documented aLVO. Further studies are needed to assess whether extending the MT time window and more aggressive LVO screening protocols mitigate this disparity.Disclosures A. Wallace: None. D.
