Valve-Sparing Root Replacement vs Composite Valve Grafting in Aortic Root Dilation: A Meta-Analysis.
CONCLUSIONS: Aortic valve sparing appears to be safe, and associated with reduced late mortality, thromboembolism/stroke and bleeding compared with composite valve grafting. Late durability is equivalent. Aortic valve sparing should be considered in patients with favorable aortic valve morphology. PMID: 31981499 [PubMed - as supplied by publisher]

Risk of Intraatrial Thrombi After Thoracoscopic Ablation in Absence of Heparin and Appendage Closure.
CONCLUSIONS: Thoracoscopic ablation of AF can be associated with a risk of left atrial appendage thrombus formation and possibly also stroke. With administration of heparin during the ablation, followed by occlusion of the left atrial appendage as a part of the procedure, this risk can be effectively reduced. Non-Vitamin K Antagonist Oral Anticoagulant vs Warfarin for Post Cardiac Surgery Atrial Fibrillation.
CONCLUSIONS: NOACs are associated with a reduction in post-operative LOS, without excess bleeding or other short-term complications, compared to warfarin. These findings support the broader use of NOACs as a safe alternative to warfarin in patients with post-cardiac surgery AF at elevated stroke risk and acceptable bleeding risk. Outcome of redo surgical aortic valve replacement in patients ≥ 80 years: results from the multicenter RECORD initiative.
CONCLUSIONS: Octogenarians undergoing S-AVR after prior cardiac surgery have similar immediate postoperative outcome to younger patients and their 5-year outcome is excellent. These data suggest that indication to TAVR should not rely only on coexistence of advanced age and history of prior cardiac surgery. PMID: 24036070 [PubMed - as supplied by publisher]

The Subclavian Intraaortic Balloon Pump: A Compelling Bridge Device for Advanced Heart Failure.
CONCLUSIONS: The SC-IABP provided excellent hemodynamic support with minimal morbidity and mortality, allowed for extensive rehabilitation, and permitted more than 90% of patients to receive their intended therapy. Therefore, SC-IABP is a compelling bridge device for patients with advanced congestive heart failure. PMID: 26228596 [PubMed - as supplied by publisher]

Factors Associated with Prolonged Survival in Left Ventricular Assist Device Recipients.
CONCLUSIONS: Preoperative diabetes and amiodarone use were associated with poor long-term survival in LVAD recipients. Development of early complications of stroke, gastrointestinal bleeding, hemolysis, or pump thrombosis was also associated with poor long-term survival. Early diagnosis and treatment of these complications may improve survival in LVAD recipients. PMID: 30316851 [PubMed - as supplied by publisher]

Reversal and Resumption of Antithrombotic Therapy in LVAD- Associated Intracranial Hemorrhage.
CONCLUSIONS: Reversal of anticoagulation appears safe following LVAD-associated intracranial hemorrhage, though inadequate reversal was common. Resumption of warfarin±antiplatelet was associated with fewer fatal and non-fatal thrombotic events compared to antiplatelets alone, though more non-fatal hemorrhage events occurred. Continuous-Flow Left Ventricular Assist Device Survival Improves with Multidisciplinary Approach.
CONCLUSIONS: Implementing a comprehensive multidisciplinary approach substantially improved outcomes for CF-LVAD recipients. PMID: 30853587 [PubMed - as supplied by publisher]

Routine Stent-Bridging to the Supraaortic Vessels in Aortic Arch Replacement - 10 year-experience
CONCLUSIONS: SAVSTEB is a comparatively simple, safe and efficacious technique to create the anastomosis between tetrabranched arch grafts and the supra-aortic arteries in the short and intermediate term. Bleeding from the anastomoses, kinking and scar associated stenosis are negligible; however, vertebral overstenting remains a critical technical issue.PMID:34186091 | DOI:10.1016/j.athoracsur.2021.05.074

A comparative study of thoracoscopic left atrial appendage clipping versus stapled resection
CONCLUSIONS: The clipping group demonstrated a higher rate of complete LAA closure compared to the stapled resection group. Close monitoring of patients with residual LAA stumps is essential. Further research with larger cohorts is needed to elucidate impact of residual LAA stump on thromboembolic events.PMID:37734642 | DOI:10.1016/j.athoracsur.2023.09.010

Hypercoagulability Markers Predict Thrombosis in Single Ventricle Neonates Undergoing Cardiac Surgery.
CONCLUSIONS: Single ventricle physiology patients are at higher risk for thrombosis compared with other neonates after cardiac surgery. Hypercoagulable panel testing may help risk stratify patients and guide patient specific anticoagulation management in the postoperative period. PMID: 23809731 [PubMed - as supplied by publisher]

Preoperative Atrial Fibrillation Increases Risk of Thromboembolic Events After Left Ventricular Assist Device Implantation.
CONCLUSIONS: Patients with preoperative AF have a lower freedom from TE events after LVAD implant. While overall late survival was not significantly reduced in these patients, refinement in anticoagulation strategies after VAD implant may be required. Thromboembolism in a patient with a mechanical mitral valve during anticoagulation with dabigatran etexilate.
We report a 57-year-old man with a mechanical heart valve who experienced acute upper limb thromboembolism during dabigatran intake. Dabigatran might be inadequate for thromboprophylaxis after mechanical valve replacement. PMID: 24182477 [PubMed - in process]

Long-Term Outcomes of the Maze Procedure Combined With Mitral Valve Repair: Risk of Thromboembolism Without Anticoagulation Therapy.
CONCLUSIONS: Adopting appropriate selection criteria for discontinuing anticoagulation therapy, the majority of patients receiving concomitant mitral repair and the Maze procedure can discontinue warfarin with excellent long-term safety profiles. PMID: 26116476 [PubMed - as supplied by publisher]
