Minimal access compared to sternotomy for aortic root and arch surgery
CONCLUSION: The partial upper sternotomy approach is safe and feasible for aortic root and arch surgery with morbidity and mortality rates similar to full sternotomy, with the advantages of less blood loss and transfusions need, faster extubation, and shorter length of hospital stay.PMID:35255771 | DOI:10.1080/00015458.2022.2050979

Long-term outcomes of eversion and conventional carotid endarterectomy: A multicenter clinical trial
CONCLUSION: ECEA is beneficial over CCEA in a long term.PMID:35321600 | DOI:10.1177/17085381221084803

Eptifibatide bridging therapy for staged carotid artery stenting and cardiac surgery: Safety and feasibility
CONCLUSIONS: For patients undergoing staged CAS-CTS, Eptifibatide bridging therapy is a viable temporary antiplatelet strategy with a favorable safety profile. This strategy enables a flexible range of time-intervals between procedures.PMID:35341420 | DOI:10.1177/17085381221084813

Physiological reno-portal bypass in liver transplantation with non-tumorous portal vein thrombosis
AbstractReno-portal anastomosis (RPA) in presence of spleno-renal shunts (SRS) is a physiological option to restore blood flow in liver transplantation with portal vein thrombosis (PVT). Diffuse splanchnic venous system thrombosis (complex PVT) is its main indication but RPA proved to be useful in selected cases of less extensive thrombosis (non-complex PVT). Up until now only two monocentric and one multicentric case series has been published on this topic in addition to few anecdotal reports. After 2014, we introduced RPA in our institution to manage some cases of complex PVT in presence of SRS. Here, we present the evol...

Ticagrelor or clopidogrel for dual antiplatelet therapy after transcarotid and transfemoral stenting of the carotid artery: are we ready for a randomized controlled trial?
In the work by Marcaccio et  al,1 the efficacy and safety of perioperative dual antiplatelet therapy (DAPT) with aspirin/ticagrelor vs aspirin/clopidogrel in patients undergoing carotid artery stenting (CAS) or transcarotid artery revascularization (TCAR) was examined using data collected from the Vascular Quality Initiative. Compared with aspirin/clopidogrel, aspirin/ticagrelor was associated with a potentially lower risk of stroke/death and bleeding complications after CAS in cases in which protamine was used but a higher risk of these outcomes in the absence of protamine.

Risk of bleeding complications per different perioperative antithrombotic regimes during carotid endarterectomy: a national registry analysis
Antithrombotic therapy is one of the cornerstones of the prevention of (recurrent) ocular or cerebral ischaemic events in patients with carotid artery stenosis. Randomized controlled trials on antithrombotic therapy for patients with minor ischaemic stroke and transient ischaemic attack (TIA) have recommended dual antiplatelet therapy (DAPT) in the three weeks following the index event. However, these trials excluded patients undergoing carotid revascularization. To date, the optimal antithrombotic therapy during the perioperative period of carotid endarterectomy (CEA) remains unclear.
