Patent foramen ovale closure versus medical therapy after cryptogenic stroke: An updated meta-analysis of all randomized clinical trials.
CONCLUSION: Patent foramen ovale closure in adults with recent cryptogenic stroke was associated with a lower rate of recurrent strokes in comparison with medical therapy alone. PMID: 29512097 [PubMed - as supplied by publisher]

Pilot of a Computerised Antithrombotic Risk Assessment Tool Version 2 (CARATV2.0) for stroke prevention in atrial fibrillation.
CONCLUSIONS: This decision support tool can help optimise the use of antithrombotic therapy in patients with AF by considering risk versus benefit profiles and rationalising treatment selection. PMID: 28070883 [PubMed - as supplied by publisher]

Antithrombotic therapy in patients with atrial fibrillation and acute coronary syndrome in the real world: data from the Berlin AFibACS Registry.
CONCLUSIONS: These data suggest that in AFibACS patients, antithrombotic treatment focussed on dual antiplatelet therapy for ACS, rather than on anticoagulation therapy for stroke prevention. Factors influencing therapy at discharge were age, renal failure, stent implantation, AFib category, and CHA₂DS₂-VASc score. During the study period, triple therapy increased for stented patients. Left atrial appendage closure for primary primary prevention during percutaneous closure of septal defects in patients with large atria but no atrial fibrillation.
CONCLUSIONS: Concomitant closure of ASD or PFO in the presence of enlarged atria and LAA for primary primary prevention appears feasible and safe but has yet to prove its justification. PMID: 28840592 [PubMed - as supplied by publisher]

The use of anticoagulants in chronic kidney disease: Common point of view of cardiologists and nephrologists.
Abstract In patients diagnosed with chronic kidney disease (CKD), atrial fibrillation (AF) is associated with an increased risk of thromboembolism and stroke. Moreover, patients with CKD - especially those in end-stage renal disease - also present an increased risk of bleeding. Oral anticoagulation is the most effective form of thromboprophylaxis in patients with AF and an increased risk of stroke. However, the underuse of these drugs was observed, mainly due to safety reasons and restricted evidence on efficacy. Much evidence suggests that non-vitamin K-dependent oral anticoagulant agents significantly reduce the...

Evaluation of the impact of warfarin's time-in-therapeutic range on outcomes of patients with atrial fibrillation in Turkey: Perspectives from the Observational, Prospective WATER Registry.
CONCLUSIONS: WATER provides insights into the anticoagulation control status of AF patients in Turkey. The quality of anticoagulation was poor. Strategies should be undertaken by the clinician and patients to improve TTR. New oral anticoagulant agents may be ideal alternatives for NVAF patients. The OPTImal duration of Dual Anti Platelet Therapy in patients receiving percutaneous coronary intervention with drug-eluting stents.
CONCLUSIONS: There was no difference in efficacy outcomes between short-term and long-term DAPT following DES, even among high-risk patients. However, longer duration of DAPT was found to be associated with increased risk of major bleeding. PMID: 26711462 [PubMed - as supplied by publisher]

Twelve-month follow-up of left atrial appendage occlusion with Amplatzer Amulet.
CONCLUSIONS: Although minimizing procedure-related complications remains challenging, LAAO with the Amplatzer Amulet device showed high procedural success and excellent LAA sealing. PMID: 28198520 [PubMed - as supplied by publisher]

Should targeted temperature management be used in cardiogenic shock patients? Systematic review and meta-analysis
CONCLUSIONS: The present analysis shows no significant benefit of TTM in patients with CS. Moreover, no statistically significant increase of the incidence of adverse effects was found. However, further randomized studies with higher sample size and greater validity are needed to determine if TTM is worth implementing in CS patients.PMID:36200546 | DOI:10.5603/CJ.a2022.0093
