Further medical experience will be required to validate these results: How Experience Based Medicine Shapes the Validity of Medical Evidence
In 1992 there appeared in the Journal of The American Medical Association a landmark article entitled Evidence-Based Medicine (EvBM): A New Approach To Teaching The Practice Of Medicine [1]. This marked the soft launch of an EvBM movement. In the ensuing years, every corner of medicine has been charged by payers, administrators, quality leaders, department heads, and academia with justifying medical practice and its value to patients through the use of evidence. However, the well-known limitations of EvBM and the very literature on which it is ‘based’ eventually spawned a backlash from practitioners defending the impor...

A conversation on how animal experimental studies allies with translational medicine
Basis science research in pediatric urology studies physiological questions in animal models to provide new data on how to treat patients. We sat down with Jens-Chrisitian Djurhuus to discuss both practical and philosophical questions on how to best perform basic science research. Initial questions such as “What type of questions are best studied in animal models?” and “Which animal model should I use?” help us answer the basic question of “What is a good research topic?” To take advantage of his experience leading a research team, we went on to discuss “How do you lead a research team?” and “How do we ma...

Development of a patient-reported outcome measure for the assessment of symptom burden in pediatric chronic kidney disease (PRO-Kid)
ConclusionsPRO-Kid is the first pediatric CKD/kidney failure-specific PRO tool to assess symptom burden. Future work should validate this tool in a larger cohort so that it may be used to improve the care of children living with CKD/kidney failure.Graphical abstractA higher resolution version of the Graphical abstract is available as Supplementary information

COVID-19 in pediatric kidney transplantation: a follow-up report of the Improving Renal Outcomes Collaborative
ConclusionsDespite the increase in testing and positive tests over this study period, the incidence of allograft loss or death related to COVID-19 remained extremely low, with allograft loss or death each occurring in  <  1% of COVID-19-positive patients and in less than <  0.1% of all transplant patients within the IROC cohort.Graphical abstractA higher resolution version of the Graphical abstract is available asSupplementary information

Variation in Care between Pediatric and Adult Patients Presenting with Nephrolithiasis to Tertiary Care Pediatric Emergency Departments in the United States (2009 – 2020)
Individuals with nephrolithiasis frequently present to the Emergency Department (ED). Safety and quality principles are often applied in pediatric EDs to children presenting with nephrolithiasis, such as limiting ionizing radiation exposure and opioid analgesics. However, it is unknown whether pediatric EDs apply these same principles to adult patients who present with nephrolithiasis. We hypothesized that adult patients would be associated with higher use of radiation-based imaging and opioid analgesics.

Monitoring of blood glucose after pediatric kidney transplantation: a longitudinal cohort study
ConclusionsUp to two thirds of pediatric KTRs (71.4%) experienced abnormal glycemic state at some point with peak incidence within the first week up to 6  months after KT. OGTT was a better tool for monitoring of glucose metabolism than FPG. Abnormal glycemic state was induced by tacrolimus and adversely affected graft function.Graphical abstractA higher resolution version of the Graphical abstract is available asSupplementary information.

Characterizing Digital Access in Pediatric Urology
Virtual medicine has the potential to improve access for underserved populations by facilitating timely remote evaluation. However, challenges in the real-world implementation of this technology may paradoxically exacerbate health disparities. We sought to characterize families ’ digital access and how social determinants of health may impact virtual medicine access within pediatric urology. We hypothesized that disadvantaged socioeconomic status would be a barrier to virtual medicine access.

Predictors of successful discontinuation of continuous kidney replacement therapy in a pediatric cohort
ConclusionsDetermining the predictors of sustained CKRT discontinuation is critical. Urine output greater than 0.5  mL/kg/h in this pediatric cohort predicted successful discontinuation. Future studies are needed to validate this threshold in disease- and age-specific cohorts and evaluate additional biomarkers of kidney injury.Graphical abstractA higher resolution version of the Graphical abstract is available asSupplementary information

Resource use and costs of transitioning from pediatric to adult care for patients with chronic kidney disease
ConclusionsA structured transition pathway for patients with CKD is resource and time –consuming due to the complexity of the disease and should be sufficiently funded.Graphical AbstractA higher-resolution version of the Graphical abstract is available asSupplementary information

Quiz Page March 2013: A Critically Ill Infant With Sepsis, Respiratory Failure, and Anasarca
A 1-month-old full-term male infant presented with decreased activity, poor appetite, and abdominal distension. Serologic testing during the first trimester of pregnancy showed that the mother was nonreactive upon rapid plasma reagin (RPR) testing, was rubella immune, and had negative results for hepatitis B and C, toxoplasma, and human immunodeficiency virus (HIV) tests. Vaginal/cervical specimens were negative for chlamydia, gonorrhea, and streptococcus group B. On admission, the infant's weight was 3.6 kg, height was 48.5 cm, rectal temperature was 94.7°F, heart rate was 150 beats/min, respiratory rate was 68 breaths/m...

Sequential use of hemoperfusion and single-pass albumin dialysis can safely reverse methotrexate nephrotoxicity
ConclusionCHP and SPAD are effective extracorporeal methods of removing methotrexate. They provide alternative treatment options for critical care nephrologists in the management of methotrexate toxicity.

Introducing the Pediatric Urology Journal Club on Twitter
The Journal of Pediatric Urology is excited to announce the Pediatric Urology Journal Club (PUJC). Beginning February 2017, JPU will sponsor monthly discussions on Twitter pertaining to peer-reviewed research articles on topics in pediatric urology. The journal club is the first of its kind in pediatric urology and one of the first in academic medicine.

Preoperative nutritional status and use of total parenteral nutrition in pediatric and adolescent patients undergoing continent urinary tract reconstruction
A nutritional assessment is a critical but often neglected aspect of a preoperative evaluation. While malnutrition is clearly associated with worse surgical outcomes in adults undergoing major abdominal surgery, a paucity of evidence is available in the pediatric population.
