Push dose epinephrine use in the management of hypotension during critical care transport.
CONCLUSIONS: Push dose epinephrine may be an effective method of temporarily resolving hypotension during the CCT of critically ill patients. In the cases where PDE was administered there was close adherence to the established protocol and adverse events were found to be rare directly following PDE administration. Further research is needed to validate these findings, establish optimal dosing, and evaluate use in non-CCT prehospital settings. PMID: 30808241 [PubMed - as supplied by publisher]

Frailty in Critical Care Medicine: A Review.
Abstract Traditional approaches to clinical risk assessment utilize age as a marker of increased vulnerability to stress. Relatively recent advancements in the study of aging have led to the concept of the frailty syndrome, which represents a multidimensional state of depleted physiologic and psychosocial reserve and clinical vulnerability that is related to but variably present with advancing age. The frailty syndrome is now a well-established clinical entity that serves as both a guide for clinical intervention and a predictor of poor outcomes in the primary and acute care settings. The biological aspects of the...

Education and training for moral and ethical decision-making at the end of life in critical care
Publication date: December 2014 Source:Trends in Anaesthesia and Critical Care, Volume 4, Issue 6 Author(s): Guglielmo Consales , Lucia Zamidei , Giuliano Michelagnoli End-of-life issues are core components of critical care. Physicians, nurses and caregivers that work in the Intensive Care Unit must know the ethical framework that guides decision-making and must be able to communicate with patients and their families in order to improve the quality of care and to reduce conflicts. A formal education in bioethics and an in-depth training in communication must be introduced in the medicine resident curricula.

The impact of a cadaver-based airway lab on critical care fellows’ direct laryngoscopy skills
This study sought to determine the impact of a cadaver-based airway lab on critical care medicine fellows’ direct laryngoscopy skills and hypothesised that fellows can improve their self-reported percentage of glottic opening (POGO) scores in cadaver models to achieve POGO scores similar to that of expert faculty. Nineteen fellows attended an airway management skills laboratory utilising five modified cadavers. Initial fellow POGO (POGOi) visualised was recorded at the onset with direct laryngoscopy. Maximum fellow POGO score with optimised direct laryngoscopy was recorded after two additional hours of teaching and also ...

Efficacy and safety of sedation with dexmedetomidine in critical care patients: A meta-analysis of randomized controlled trials
Conclusions and relevance In this first meta-analysis including only randomized controlled trials related to ICU patients, dexmedetomidine was associated with a 48h reduction in ICU length of stay, mechanical ventilation duration and delirium occurrence despite a significant heterogeneity among studies. Dexmedetomidine was also associated with an increase in bradycardia and hypotension.
