The Fundamentals of Resident Dismissal.
Abstract Residents have the rights and responsibilities of both students and employees. Dismissal of a resident from a training program is traumatic and has lasting repercussions for the program director, the faculty, the dismissed resident, and the residency. A review of English language literature was performed using PUBMED and OVID databases, using the search terms, resident dismissal, resident termination, student dismissal, student and resident evaluation, legal aspects of education, and remediation. The references of each publication were also reviewed to identify additional appropriate citations. If the Jus...

What ’s In The Manager’s Amendment To AHCA?
On March 20, 2017, the Republican House leadership released a manager’s amendment to the American Health Care Act. This amendment is intended to respond to critics of the original AHCA from the right and from the middle. It is less clear that it responds to concerns raised by the Congressional Budget Office report on the original AHCA, as will be discussed later. The original AHCA, as it passed the House Budget Committee, (Budget Committee Report) contained provisions that would: Phase out the ACA’s Medicaid expansion; Impose a per capita cap on Medicaid going forward; Eliminate the ACA’s Prevention and Public Healt...

“Value-Pricing” of Drugs and Pharmaceutical Innovation
By UWE REINHARDT In a fascinating paper on drug pricing, Ana D. Vega and her five co-authors trace increases in the price of brand-name and generic drugs in the U.S. during the period 2012-15, using the national average drug acquisition costs (NADAC) data made public by the Centers for Medicare and Medicaid Services (CMS). These acquisition costs are the prices that retail community pharmacies pay to acquire medicines, usually from a wholesaler. The tables in the paper show that the top 50 increases in the price of generic drugs over the three-year period ranged from a “low” of 448% to a high of 18,808%. For branded dr...

Scott Gottlieb And The Goldilocks Theory Of Bringing Change To The FDA
With the nomination of Dr. Scott Gottlieb to head the U.S. Food and Drug Administration (FDA) headed to the Senate floor, we can expect another Goldilocks debate. Like the heroine of the classic tale who famously tested the three bowls of porridge to find one too hot, one too cold, and one “just right,” participants will debate whether today’s FDA is too lenient, too tough, or just right in reviewing new prescription drugs. That Dr. Gottlieb is qualified for the role he has been nominated for appears beyond question. What critics take issue with is whether his extensive experience ties him too closely to the entities...

Selection and Implementation of an Electronic Health Record
Electronic health records (EHRs) are now the standard of practice in most communities, because of transition to reimbursement that increasingly focuses on risk sharing and quality measurement, and government EHR incentive programs. The selection and implementation of an EHR is one of the most important decisions a practice faces. Organizing the search for an EHR that fits a practice, negotiating a contract, planning and successfully implementing an EHR are best accomplished with a well-informed, strong, multidisciplinary team using project management techniques.

How CMS Undermines Pioneer ACOs and What to do About It
By KIP SULLIVAN In my first post  in this three-part series, I documented three problems with Pioneer ACOs: High churn rates among patients and doctors; assignment to ACOs of healthy patients; and assignment of so few ACO patients to each ACO doctor that ACO “attributees” constitute just 5 percent of each doctor’s panel. I noted that these problems could explain why Medicare ACOs have been so ineffective. These problems are the direct result of CMS’s strange method of assigning patients to ACOs. Patients do not decide to enroll in ACOs. CMS assigns patients to ACOs based on a two-step process: (1) CMS first determ...

Navigating South Korea ’s Healthcare Paradox
In recent years, we have witnessed an explosion of interest among our pharmaceutical clients in conducting research in South Korea. Some emerging market analysts have suggested a ‘K’ should be added to BRIC, reflecting the country’s burgeoning economic status and its increasingly inclusion in global studies, not just regional Asia Pacific or emerging market studies. Indeed, there is ongoing debate about whether South Korea can still be considered ‘emerging’ as it i s a growing player in the pharmaceutical industry in its own right.   As well as looking at South Korea ’s healthcare outlook in more detail, in ...
