Alterations in rat brain modular organization during unconsciousness are dependent on communication efficiency and metabolic cost
We examined alterations in brain modular variability during loss of consciousness from mild sedation to deep anesthesia. We also investigated the relationships between modular variability with glucose metabolism and functional connectivity strength as well as their alterations during unconsciousness. We observed that modular variability increased during loss of consciousness. Critically, across-individual modular variability is oppositely associated with functional connectivity strength and cerebral metabolism, and with deepening dosage of anesthesia, becoming increasingly dependent on basal metabolism over functional conn...

What Can We Learn from the Envision Bankruptcy?
This study ignited a firestorm of press criticism and was followed by an aggressive lobbying and PR campaign funded by United and other large commercial payers  aimed at restricting balance billing by firms like Envision.  This campaign culminated in the Dec 2020 Congressional passage of the No Surprises Act, which effectively ended balance billing and subjected thousands of Envision’s out-of-network bills to an arbitration process. NSA went into effect in January 2022.   Ironically, days prior to its Chapter 11 filing, Envision won a $91 million judgment from an arbitration panel against United fo...

Office-Based Minor Surgery: An Ever-Increasing Challenge Not Only for the Dutch General Practitioner
CONCLUSIONS: The increasing demand for OBMS in primary care centers requires more attention to training aspects, the existing fear, adequate reimbursement for special accreditation, and collaboration and organization. We expect that addressing these factors will result in the strengthening of primary care, improving patient safety, lower referral rates, and a reduction in health care costs.PMID:36656884 | DOI:10.22454/FamMed.55.740827

The Macro View – Health, Economics, and Politics and the Big Picture. What I Am Watching Here And Abroad.
The objective most consistent with recent operations is to conquer Luhansk, Donetsk, and Kherson, with a view to their eventual annexation and Russification. But not only are they some way from achieving that (w ith much of Donetsk still in Ukrainian hands and the Russia position in Kherson highly contested) it would also require an explicit Ukrainian surrender for it to serve as the basis for a declaration of victory. That will not be forthcoming.-----https://www.afr.com/world/europe/how-britain-giggled-its-way-into-crisis-20220710-p5b0giHow Britain giggled its way into crisisBoris Johnson has exposed the costs of Britain...

Global distribution of publications in anesthesiology : A  bibliometric analysis from 1999 to 2018
CONCLUSION: Regarding quantity trend, the output of global production in anesthesiology increased continuously as the number of articles from the high-output countries showed an increasing trend; however, there was still a gap between developing and developed countries in research quality. High-quality research should be encouraged in developing countries.PMID:34003302 | DOI:10.1007/s00101-021-00969-w

Combining Dedicated Online Training and Apprenticeships in the Field to Assist in Professionalization of Humanitarian Aid Workers: a 2-year Pilot Project for Anesthesia and Intensive Care Residents Working in Resource Constrained and Low-income Countries
Introduction In 2010 there was an estimated total 274,000 humanitarian workers worldwide with a growth rate of 4% from the previous year. Although the system is still dominated, in terms of operational presence and resource share, by a small group of NGOs and international organizations whose combined humanitarian expenditure in 2010 exceeded $2.7 billion, there are an estimated 4,400 NGOs worldwide that undertake humanitarian action on an ongoing basis1 . This heterogeneous and fragmented nature of the humanitarian sector has hindered its ability to efficiently respond to crises worldwide2 , and concerns have been express...
