Evaluating the Impact and Cost-Effectiveness of Statin Use Guidelines for Primary Prevention of Coronary Heart Disease and Stroke.
Conclusions -At a population level, the ACC-AHA guideline for expanded statin use for primary prevention is projected to treat more people, save more lives, and cost less compared with ATP III, in both men and women. Whether individuals benefit from long-term statin use for primary prevention depends more on the disutility associated with pill burden than their degree of cardiovascular risk. Incident Cardiovascular Disease Among Adults with Blood Pressure < 140/90 mm Hg.
Conclusions -While higher BP levels are associated with increased CVD risk, in the modern era, the majority of incident CVD events occur in US adults with SBP/DBP<140/90 mmHg. Although absolute risk and cost-effectiveness should be considered, additional CVD risk reduction measures for adults with SBP/DBP<140/90 mmHg at high risk for CVD may be warranted. Secondary Prevention of Cardiovascular Disease in Patients with Type 2 Diabetes: International Insights from the TECOS Trial.

Cholesterol Efflux Capacity, HDL Particle Number, and Incident Cardiovascular Events. An Analysis from the JUPITER Trial (Justification for the Use of Statins in Prevention: An Intervention Trial Evaluating Rosuvastatin).
Conclusions -In JUPITER, cholesterol efflux capacity was associated with incident CVD in individuals on potent statin therapy but not at baseline. For both baseline and on-statin analyses, HDL particle number was the strongest of four HDL-related biomarkers as an inverse predictor of incident events and biomarker of residual risk. Medical Therapy for Secondary Prevention and Long-Term Outcome in Patients with Myocardial Infarction with Non-Obstructive Coronary Artery (MINOCA) Disease.
Conclusions -The results indicate long-term beneficial effects on outcome in patients with MINOCA of treatment with statins and ACEI/ARBs, a trend toward a positive effect of beta-blocker treatment, and a neutral effect of DAPT. Achievement of Dual Low-Density Lipoprotein Cholesterol and High-Sensitivity C-Reactive Protein Targets More Frequent With the Addition of Ezetimibe to Simvastatin and Associated With Better Outcomes in IMPROVE-IT.
CONCLUSIONS: Significantly more patients treated with ezetimibe/simvastatin met prespecified and exploratory dual LDL-C and hs-CRP targets than patients treated with simvastatin alone. Reaching both LDL-C and hs-CRP targets was associated with improved outcomes after multivariable adjustment. Clinical Effectiveness of Statin Therapy after Ischemic Stroke: Primary Results from the Statin Therapeutic Area of the Patient-Centered Research into Outcomes Stroke Patients Prefer and Effectiveness Research (PROSPER) Study.

Utility of Non-Traditional Risk Markers in Individuals Ineligible for Statin Therapy According to the 2013 ACC/AHA Cholesterol Guidelines.
CONCLUSIONS: -In this generally low-risk population sample, a large proportion of ASCVD events occurred among adults with a 10-yr. cPCE risk <7.5%. We found that the CAC, hsCRP, FH and ABI recommendations by the ACC/AHA cholesterol guidelines (Class IIB) identify small subgroups of asymptomatic population with <7.5% 10 yr. cPCE but with observed ASCVD event rates higher than 7.5% who may warrant statin therapy considerations. Risk of Intracranial Hemorrhage from Statin Use in Asians: A Nationwide Cohort Study.
CONCLUSIONS: -Generally, no association was observed between cumulative statin use and risk of ICH among subjects without a prior history of stroke. Optimal Medical Therapy Improves Clinical Outcomes in Patients Undergoing Revascularization With Percutaneous Coronary Intervention or Coronary Artery Bypass Grafting: Insights From the Synergy Between Percutaneous Coronary Intervention With TAXUS and Cardiac Surgery (SYNTAX) Trial at the 5-Year Follow-Up.
CONCLUSIONS: The use of OMT remains low in patients with complex coronary disease requiring coronary intervention with percutaneous coronary intervention and even lower in patients treated with CABG. Lack of OMT is associated with adverse clinical outcomes. Targeted strategies to improve OMT use in postrevascularization patients are warranted.
