Prevalence and Mortality Risk of Neurological Disorders during the COVID-19 Pandemic: An Umbrella Review of the Current Evidence
Conclusion: Our results show that the preva lence and mortality risk may increase in some neurological diseases during the COVID-19 pandemic. Future studies should elucidate the precise mechanisms for the link between COVID-19 and neurological diseases, determine which patient characteristics predispose them to neurological diseases, and cons ider potential global patient management.

Age gap between stroke patients included in randomized clinical trials of acute revascularization therapy and those in population-based studies. A review
Background: The ongoing aging population in high-income countries is responsible for a dramatic rise in the number of elderly stroke patients in whom many questions remain regarding the use of acute revascularization therapy. This review aimed to compare stroke patients from population-based studies to those included in RCTs in terms of age. Summary: Population-based incidence studies conducted in high income that complied with the defined quality criteria were reviewed (period 1985-2020). RCTs of acute ischemic stroke therapy including intravenous thrombolysis (IVT) with either alteplase or tenecteplase and mechanical th...

Predictive Value of Pediatric Stroke Diagnoses in Administrative Data: A Systematic Literature Review
Conclusion Caution is recommended in the use and interpretation of non-validated administrative data for pediatric stroke. Data on the PPV and sensitivity of pediatric stroke diagnoses in administrative data remain limited and are only available from high-income countri es.

Impact of Smoking on Functional Prognosis after Ischemic Stroke according to Deprivation: A Prospective Cohort Study
Conclusion: The effect of smoking on functional prognosis after ischemic stroke varied according to deprivation status, with poorer outcomes observed in nondeprived patients.Neuroepidemiology

Epidemiology of first and recurrent ischemic stroke in Sweden 2010 –2019 – A Riksstroke study
Conclusion: Whereas both first-ever and recurrent IS rates declined in Sweden between 2010 and 2019, the proportional decline was almost double for recurrent IS than for first-ever IS and most pronounced in the elderly. Increased use of secondary preventive drugs, in particular anticoagulants in atrial fibrillation, appears to have contributed, but further studies on precise causes for the decline in recurrent IS are needed.

Real-world effectiveness of lipid-lowering medications on outcomes after stroke: potential implications of the new-user design
Introduction: Observational studies are increasingly being used to provide evidence on the real-world effectiveness of medications for preventing vascular diseases, such as stroke. We investigated whether the real-world effectiveness of treatment with lipid-lowering medications after ischemic stroke is affected by prevalent user bias. Methods: An observational cohort study of 90-day survivors of ischemic stroke, using person-level data from the Australian Stroke Clinical Registry (2012-2016; 45 hospitals) linked to administrative (pharmaceutical, hospital, death) records. Use of, and adherence to (proportion of days covered

Within-Visit Blood Pressure Variability and All-Cause and Stroke Mortality: Thai Epidemiologic Stroke (TES) Study
This study included 19,614 participants aged 45-80 years, who were free o f stroke and had three blood pressure (BP) measurements, taken one minute apart, at baseline. Within-visit systolic blood pressure (SBP) and diastolic blood pressure (DBP) variability were expressed as the maximum absolute difference (MAD) between any two readings among the three repeated sequentia l measurements of SBP and DBP respectively. The participants were followed up for mortality. Cox regression analysis was used to identify the association of within-visit BPV with all cause and stroke mortality. Hazard ratio (HR) and 95% confidence interval...

Role of Proxy Respondents in International Stroke Research: Experience of the INTERSTROKE Study
Conclusion Use of proxy respondents enhances the generalisability of international research studies of stroke, by increasing representation of women, patients with severe stroke, older age, and lower education.

Trends in Stroke Burden in Central and Eastern Europe from 1990 –2019
Conclusions: The burden of stroke contributes to the European health gap through preventable premature stroke deaths. There are some very successful countries in stroke burden management from both Central (Slovenia, Czech Republic, and Hungary) and East Europe (Estonia), suggesting that closing the health gap between East and West is a realistic aim.

Incidence and Severity of Intracerebral Hemorrhage on Oral Anticoagulation and Antiplatelet Therapy
This study aimed to assess the occurrence of intracerebral hemorrhage (ICH) in patients using OA or AP registered in the JOINVASC between 2015 and 2020. Higher mortality among OA or AP patients was seen, with a statistically significant difference in mortality within 90 days (38% versus 25%, p=0.025). For patients with OA at the time of ICH, high stroke severity was observed, with a 71% mortality rate. Managing bleeding episodes on OA and AP is key in the effort to prevent cardiovascular death and disability, and improved access to DOAC and reversal agents is in need for this patient population.
