136 Coincidential hydrocephalus in twins with CMT1X
Discussion Despite the finding of hydrocephalus in both affected twins, further family evaluation confirmed the hydrocephalous not to segregate with the GJB1 mutation. This highlights the importance of detailed phenotype segregation studies before diagnosing an atypical phenotype with genetic diseases. 266 A drowsy patient: beyond the familiar territory
A 72-year-old man was brought to the hospital after he was found unresponsive at home by his wife who reported that he has been sleepy in the last few days. He has a background of hypertension, myocardial infarct, and osteoarthritis. His medications include anti-hypertensive and morphine sulphate. 011 Progressive myoclonic epilepsy due to rare mitochondrial ND6 mutation, m.14487T>C
We report the case of a 43-year old man with adult-onset drug-resistant epilepsy, cortical myoclonus and bilateral optic neuropathies. Serial MRI scans over a two year period demonstrated the interval devel- opment of bihemispheric stroke-like lesions. Giant somatosensory evoked potentials and short-duration myoclonic jerks with craniocaudal spread on surface-EMG were consistent with cortical myoclonus. Radiology ruse: dural venous sinus cyst masquerading as venous sinus thrombosis presenting with headache and stroke
Conclusions Radiological differentiation between venous sinus thrombosis and dural venous sinus cyst is clinically important to avoid unnecessary anticoagulation and associated risks. Final management included propranolol for migraine prophylaxis; aspirin for acute migraine; aspirin for stroke prevention; vascular risk factor management; and cessation of triptans.

Artery of percheron occlusion: a rare cause of coma
Conclusions AOP occlusion is a rare cause of coma and should be suspected in patients who present with acute loss of consciousness, once other common aetiologies are excluded. This case raises pertinent questions regarding the indications and benefits of thrombolysis and/or clot retrieval in such patients.

Cerebral glucose metabolism and cognition in newly diagnosed Parkinson's disease: ICICLE-PD study
Conclusions Reductions in cortical FDG metabolism were present in newly diagnosed PD, and correlated with performance on neuropsychological tests. A reduced baseline parietal metabolism is associated with risk of cognitive decline and may represent a potential biomarker for this state and the development of PD dementia.

Cerebral microbleeds and neuropsychiatric symptoms in an elderly Asian cohort
Conclusions The presence of multiple CMBs is associated with high global neuropsychiatric disorder burden, in particular symptoms of depression and disinhibition. Future studies are recommended to investigate the importance of CMBs in the pathogenesis and longitudinal progression of neuropsychiatric disorders in the general elderly population.

Prognostic significance of delayed intraventricular haemorrhage in the INTERACT studies

Neck vessel dissection following whiplash-type neck injury
Arterial dissection is a recognised complication of neck manipulation and trauma. In a systematic audit of 148 complex medicolegal whiplash cases 131 had only neck pain, 8 had radiculopathic symptoms, and 9 had symptoms and signs suggesting an arterial dissection of a neck vessel with presumed secondary thromboembolism. Their ages ranged from 28–55. Three had hemipareses supported by scan abnormalities, and one had a probable posterior fossa stroke. Five cases had prolonged symptoms without imaging abnormalities. The effect of minocycline on neuroinflammation after brain trauma
Conclusion Progressive volume loss in white matter with high microglial activation indicates on-going neurodegeneration. Minocycline reduced [11C]PBR28 binding, suggesting a treatment effect on microglial activation. These findings support the case for further clinical evaluation of minocycline as a treatment for neurodegeneration after TBI.

Conclusions MM is associated with multiple recurrences, progressive arteriopathy and poor outcome in half of cases, especially with AIS presentation and posterior circulation involvement. Recurrent AIS is rare after surgery but surgery was not a determinant of overall outcome; this could reflect surgical selection criteria or the patients' clinical state at referral.
