E-108 Endovascular treatment of patients with acute ischemic stroke due to underlying arterial dissection: a nationwide analysis
ConclusionsDespite a similar prevalence among patients admitted for AIS, CA dissections are associated with a higher rate of endovascular intervention and poorer short-term outcomes compared to VA dissections. This study provides essential epidemiologic and outcome data for the cerebrovascular neurosurgeon.Abstract E-108 Table 1Comparison of clinical characteristics and outcomes among patients presenting with acute ischemic stroke due to carotid or vertebral artery dissections Characteristic Vertebral Artery Dissection, N = 2,562 Carotid Artery Dissection, N = 2,789 p-value1 Age (years), Mean (SD) 53 (17) 57 (17) <0.00...

E-019 tcar
ConclusionsOur analysis of a single-center cohort demonstrates that TCAR is a safe and effective treatment for symptomatic carotid stenosis, with a low perioperative stroke risk. The present study’s findings concur with the combined adverse outcome rate of 3.5% reported in the ROADSTER trial.Disclosures A. Bajaj: None. S. Monje: None. C. Ayala: None. J. Poggi: None. E. E-094 Trends in endovascular treatment of cervical artery dissections: a nationwide analysis
ConclusionsDespite an increase in the overall use of endovascular therapy for the treatment of cervical artery dissection in patients with ischemic stroke or TIA, there was no significant association with early clinical outcomes. Future studies looking at long term outcomes including stroke-recurrence are warranted.Disclosures M. Bouslama: None. C. Zhang: None. H. E-190 Trends in admissions for intracranial dissections in the United States
ConclusionThe prevalence of uIADs among hospitalized patients is scarce, and only 2% of craniocervical dissection-related AIS is due to uIAD. Compared to IAs, patients were more likely younger and male, and uIAD more commonly led to acute ischemic stroke and motor deficits. The trend in age remained stable across the four years analyzed, while the proportion of females increased. There was no trend in the proportion of races among uIADs, however.Disclosures W. Surgical Technique for Carotid Endarterectomy: Current Methods and Problems.
Authors: Uno M, Takai H, Yagi K, Matsubara S Abstract Over the last 60 years, many reports have investigated carotid endarterectomy (CEA) and techniques have thus changed and improved. In this paper, we review the recent literature regarding operational maneuvers for CEA and discuss future problems for CEA. Longitudinal skin incision is common, but the transverse incision has been reported to offer minimal invasiveness and better cosmetic effects for CEA. Most surgeons currently use microscopy for dissection of the artery and plaque. Although no monitoring technique during CEA has been proven superior, multiple mon...

E-049 Accuracy of non-invasive and invasive imaging in identifying ica occlusion: a comparative study in 65 patients
Conclusion Our result suggests that there is a lack of correlation between the imaging findings of carotid occlusion on imaging compared with catheter angiography of the ICA during the intervention in acute stroke treatment. Thus, endovascular catheterization of the ICA is often required for the exact characterization and localization of the carotid occlusion and to determine if a dissection is the underlying pathology in such cases. Acute carotid artery stenting in symptomatic high-grade cervical carotid artery stenosis
Abstract The safety and efficacy of emergency carotid artery stenting (CAS) for patients with acute ischemic stroke resulting from internal carotid artery stenosis are not established. In this retrospective study, we evaluated outcomes for CAS performed within 2 weeks of acute ischemic stroke for 16 patients treated between December 2009 and February 2014. Cases of internal carotid artery occlusion, internal carotid dissection, or intracranial major arterial trunk occlusion were excluded. Five patients were treated with CAS during the hyperacute phase (within 24 h of stroke onset), three in the advanced phase (w...

Self-Expanding Stent Placement for Anterior Circulation Intracranial Artery Dissection Presenting With Ischemic Symptoms
BACKGROUND: The prognosis of ischemic anterior circulation intracranial dissection (AC-ICD) is poor and its optimal management is still controversial. OBJECTIVE: To evaluate the safety and efficacy of a self-expanding stent for ischemic AC-ICD. METHODS: Eight patients (mean age, 36 years) underwent self-expanding stenting for ischemic AC-ICD. Imaging findings of ischemic AC-ICD, the reason for stenting, and the clinical and angiographic outcomes were retrospectively evaluated. RESULTS: AC-ICD involved intracranial internal carotid artery to middle cerebral artery (MCA) in 2, intracranial internal carotid artery alone in 3,...

E-007 Unusual Two Cases of Dissecting Cervical Internal Carotid Artery Pseudo-aneurysm with Symptomatic Intracranial Aneurysm
Conclusions In the evaluation of suspected intracranial aneurysm who present with headache with neck pain, a catheter cerebral angiogram not only will clearly define the correct morphology of intracranial aneurysm but also will appropriately identify the presence of life threatening cervical ICA abnormalities. Additionally, presence of an unusual cervical ICA anatomy with DP may warrant further evaluation of intracranial blood vessel of same territory. Using endovascular strategies, both extracranial and intracranial DP could be treated. However, long-term follow-up and outcome is unknown. Further studies are required. Di...

E-009 Dissecting Ruptured and Symptomatic Intracranial Pseudo-aneurysm who Underwent Endovascular Repair-A Case Series
Conclusions Our series demonstrates that dissecting intracranial aneurysm is predominantly present in the Petro-Cavernous junction of the ICA. Most of them could be treated successfully with good clinical outcome using stent-assisted coiling, stent-remodeling or primary coiling. However, the recurrence rate is extremely common; therefore close early follow-up is necessary for all cases especially those located in Petro-cavernous junction of the ICA. Further long-term follow-up study is required. Disclosures Y. Lodi: None. V. Reddy: None. A. Devasenapathy: None. J. Chou: None. K. Shehades: None. K. Sethi: None. D. Galyon: ...

Endovascular treatment of acute carotid occlusion
Conclusions: Revascularization of the cervical carotid occlusion prior to treatment of the intracranial occlusion led to increased rates of recanalization in patients with tandem extracranial and intracranial occlusions. Whether a clinical benefit can be consistently derived likely relies on other factors, including the evaluation of cerebral perfusion.
