The Relationship between Glucose Control & Cognitive Function in People with Diabetes after a Lacunar Stroke.
CONCLUSION: This analysis of lacunar stroke patients with diabetes demonstrates a relationship between A1C and change in cognitive scores over time. Intervention studies are needed in order to delineate if better glucose control could slow the rate of cognitive decline in this high risk population. Proportion of undercarboxylated osteocalcin and serum P1NP predict incidence of myocardial infarction in older men.
CONCLUSIONS: Areduced proportion of undercarboxylated osteocalcin or higherP1NPare associated with increased incidence of MI. UcOC/TOC ratio and P1NP predict risk of MI but not stroke, in a manner distinct from CTX. Further studies are needed to investigate potential mechanisms by which bone turnover markers related to metabolic risk and to collagen formation could modulate cardiovascular risk. Pioglitazone and Risk for Bone Fracture: Safety Data from a Randomized Clinical Trial.
CONCLUSIONS: Fractures affected 8.8% of placebo-treated patients within five years after an ischemic stroke or TIA. Pioglitazone increased the absolute fracture risk by 1.6%-4.9% and the relative risk by 47-60%, depending on fracture classification. Our analysis suggests that treatments to improve bone health and prevent falls may help optimize the risk/benefit ratio for pioglitazone. PMID: 27935736 [PubMed - as supplied by publisher]

Impact of age-adjusted Insulin-like Growth Factor I on major cardiovascular events after acute myocardial infarction.
CONCLUSIONS: Low IGF-I score is associated with increased risk of all cause-death, recurrent MI and stroke in AMI patients. Whether patients treated by IGF-I axis inhibitors have a specific clinical course after AMI would be worth studying. PMID: 25699636 [PubMed - as supplied by publisher]

Insulin-like Growth Factor 1 and Risk of Cardiovascular Disease: Results from the UK Biobank Cohort Study
CONCLUSION: This study indicates that both low and high levels of circulating IGF-1 are associated with increased risk of CVD in general population. These results highlight the importance of monitoring IGF-1 status on cardiovascular health.PMID:36810801 | DOI:10.1210/clinem/dgad105

Insulin resistance and beat-to-beat cardiovascular dynamics: a constant relationship across different body mass index and blood pressure categories.
Conclusions: These findings indicate that even small increases in HOMA index (not necessarily in the range to define insulin resistance) may induce significant changes on indices of cardiovascular function even in normal weight and normotensive subjects, emphasizing the importance of insulin resistance at an early stage of the cardiovascular risk continuum. Factors Associated with Cardiovascular Events in Patients with Type 2 Diabetes and Acute Myocardial Infarction.
CONCLUSIONS: Postprandial glycemia should be considered a potential target in trials to reduce CV morbidity and mortality in type 2 diabetes mellitus. PMID: 26595100 [PubMed - as supplied by publisher]

Mortality and Other Important Diabetes-Related Outcomes With Insulin vs Other Antihyperglycemic Therapies in Type 2 Diabetes.
Conclusions: In people with T2DM, exogenous insulin therapy was associated with an increased risk of diabetes-related complications, cancer, and all-cause mortality. Differences in baseline characteristics between treatment groups should be considered when interpreting these results. PMID: 23372169 [PubMed - in process]

Sitagliptin Reduces Inflammation and Chronic Immune Cell Activation in HIV+ Adults with Impaired Glucose Tolerance.
CONCLUSION: Sitagliptin had beneficial systemic and adipose anti-inflammatory effects in cART-treated HIV+ adults with impaired glucose tolerance. Large scale, long-term studies should determine if sitagliptin reduces cardiovascular risk and events in HIV+ adults. PMID: 25938633 [PubMed - as supplied by publisher]

A Diet Rich in Medium-chain Fatty Acids Improves Systolic Function and Alters the Lipidomic Profile in Patients with Type 2 Diabetes: a Pilot Study.
CONCLUSIONS: Although a diet high in MCFAs does not change cardiac steatosis, our findings suggest that the MCFA-rich diet alters the plasma lipidome and may benefit or at least not harm cardiac function and fasting insulin levels in humans with type 2 diabetes. Larger, long-term studies are needed to further evaluate these effects in less controlled settings. PMID: 26652763 [PubMed - as supplied by publisher]

Cardiovascular effects of pioglitazone or sulphonylureas according to pretreatment risk: moving towards personalized care.
CONCLUSIONS: It is possible to identify patients with type 2 diabetes early in the stage of their disease and largely free from evident cardiovascular disease, in whom add-on pioglitazone to metformin confers cardiovascular protection as compared to sulphonylureas. Menopausal Hormone Therapy and Cardiovascular Disease: The Role of Formulation, Dose, and Route of Delivery.
CONCLUSIONS: Although HT continues to have an important role in menopause management, it is not recommended for primary or secondary CVD prevention. Different formulations, doses, and routes of delivery of HT have different effects on cardiometabolic markers and risks of clinical CVD events. However, long-term trials evaluating clinical outcomes with transdermal and other alternate HT regimens are limited. PMID: 33506261 [PubMed - as supplied by publisher]

Causes and Consequences of Polycystic Ovary Syndrome: Insights from Mendelian Randomization
CONCLUSIONS: The accumulation of genome-wide association studies in PCOS has enabled multiple MR analyses identifying factors that may cause PCOS or be caused by PCOS. This knowledge will be critical to future development of measures to prevent PCOS in girls at risk as well as prevent complications in those who have PCOS.PMID:34669940 | DOI:10.1210/clinem/dgab757
