Technical improvements in carotid revascularization based on the mechanism of procedural stroke.
In conclusion, this review provides an overview of the pathophysiological mechanism of stroke following carotid revascularization (both CAS and CEA) and of the technical improvements that have contributed to reducing this stroke risk. PMID: 30827087 [PubMed - as supplied by publisher]

Left atrial appendage occlusion for stroke prevention in patients with atrial fibrillation: a systematic review and network meta-analysis of randomized controlled trials.
CONCLUSIONS: LAA occlusion appears to preserve the benefits of oral anticoagulation therapy for stroke prevention in patients with AF, but the current evidence is of low quality. PMID: 28215062 [PubMed - as supplied by publisher]

Left atrial appendage occlusion for stroke prevention in atrial fibrillation: multicentre experience with the AMPLATZER Cardiac Plug.
Conclusions: In this multicentre study, LAAO with the ACP showed high procedural success and a favourable outcome for the prevention of AF-related thromboembolism. Modification in antithrombotic therapy after LAAO may result in reduction of bleeding events. PMID: 25604089 [PubMed - as supplied by publisher]

Left atrial appendage occlusion with the AMPLATZER ™ Amulet™ device: one-year follow-up from the prospective global Amulet Registry.
CONCLUSIONS: In the global prospective Amulet Registry of patients at high-risk of stroke and bleeding, the annualised ischaemic stroke rate was 2.9%. The LAA was sealed in 98.4% after 1-3 months and device related thrombus was observed in 1.7% of cases with only a minority of all patients on anticoagulation treatment. Transcatheter left atrial appendage occlusion in patients with atrial fibrillation and a high bleeding risk using aspirin alone for post-implant antithrombotic therapy.
CONCLUSIONS: LAAO with the ACP or Amulet was safely performed with ASA monotherapy after implantation without an increased risk of device-related thrombosis or stroke. PMID: 27973336 [PubMed - as supplied by publisher]

10-year results of On-X bileaflet mechanical heart valve in the aortic position: low target INR regimen in Japanese
ConclusionsAVR using On-X valve with low target INR regimen and low-dose aspirin resulted in a significantly low risk of bleeding.

Ticagrelor with and without aspirin in patients with a prior coronary artery bypass graft undergoing percutaneous coronary intervention: the TWILIGHT-CABG study
CONCLUSIONS: In high-risk patients with prior CABG, ticagrelor monotherapy reduced bleeding without compromising ischaemic outcomes compared with ticagrelor plus aspirin.PMID:35979636 | DOI:10.4244/EIJ-D-22-00319

232 * early anticoagulation therapy after bioprosthetic aortic valve implantation: comparing warfarin versus aspirin
Conclusions: This is to date the largest randomized trial comparing warfarin to aspirin as early anticoagulation therapy after implantation of bioprosthetic aortic valves. The results are not conclusive, but aspirin therapy seems as safe as warfarin therapy, and with significantly fewer gastrointestinal bleeding events, three months after surgery.

Results of percutaneous closure of patent foramen ovale with the GORE ® Septal Occluder
Conclusion. PFO closure with the GSO is accompanied by a high technical success rate and closure rates similar to other currently used devices. The incidence of atrial fibrillation was higher than reported with most other devices. This may be a chance finding but warrants further investigation in larger trials. © 2013 Wiley Periodicals, Inc.

019 * is antiplatelet therapy required in heartmate ii patients? preliminary results from the european trace study
Conclusion: This preliminary analysis of the observational TRACE-EU study suggests that managing HMII patients with a vitamin K antagonist without aspirin may help to reduce the incidence of major bleeding without increasing the risk of thrombo-embolic events including ischaemic stroke and device thrombosis. Further prospective studies are needed.

Eptifibatide infusion versus placebo in high risk patients with non-st segment elevation acute coronary syndromes managed with urgent coronary artery bypass graft surgery. a prospective multicenter randomized placebo-controlled clinical trial.
CONCLUSION: Preoperative use of eptifibatide vs. placebo is linked to significantly reduced 12--month MACCE rate in patients with NSTE--ACS requiring urgent CABG, while it simultaneously seems not to confer a greater risk of postoperative bleeding. PMID: 25394877 [PubMed - as supplied by publisher]

Simultaneous Carotid Artery Stenting and Heart Surgery: Expanded Experience of Hybrid Surgical Procedures
Conclusions Same-day hybrid approach appeared safe in terms of early and long-term results not only for CAS and isolated CABG but also for CAS and noncoronary isolated procedures. In complex cases, the rate of stroke and myocardial infarction seemed low; in-hospital mortality, as expected, was higher. Long-term survival appeared similarly satisfactory, thus confirming the hybrid approach as a valid therapeutic option for all patients with significant internal carotid artery stenosis associated with coronary and other cardiac lesions, at least for noncomplex cases.

Clinical outcomes associated with per ‐operative discontinuation of aspirin in patients with coronary artery disease: A systematic review and meta‐analysis
Conclusion: Our analysis suggests that planned short‐term discontinuation in the appropriate clinical setting appears to be safe in the correct clinical setting with no increased risk of thrombotic events and with a decreased risk of bleeding. © 2016 Wiley Periodicals, Inc.
