A case of Lemierre's syndrome causing panhypopituitarism, cavernous sinus thrombosis, ischaemic stroke and pulmonary embolism
We present the case of a previously well 19-year-old man, who presented acutely unwell with meningitis and sepsis. Fusobacterium necrophorum was isolated from peripheral blood cultures and identified on cerebrospinal fluid with 16S rDNA Polymerase Chain Reaction (PCR). Imaging demonstrated internal jugular vein thrombosis with subsequent cavernous venous sinus thrombosis. Pituitary function tests were suggestive of panhypopituitarism. 230 Sydenhams chorea in an elderly patient
79 year old gentleman presented with multiple involuntary limb movements, initially referred to the acute medical unit as a seizure. After initial triage, he was managed as a dystonia. Patient was seen by ITU, and neurology opinion was requested. On consultant review, the ‘dancing type’ involuntary movements in limbs and in the face pointed towards a generalised chorea with mild athetosis of the left side, with expulsive speech. Amoxicillin/clavulanic acid-associated severe neutropenia
A 72-year-old man presented with an abnormal blood count and was admitted. His history included hypertension and hyperlipidaemia with an old myocardial infarction and mild stroke; liver cirrhosis due to non-alcoholic steatohepatitis (NASH) with hypersplenism and oesophageal varices but no ascites, oedema or bleeding; and benign prostatic hypertrophy. Bacteriological and therapeutic features of lung abscess
Conclusion: The identification of the responsible bacteria in lung abcess remains difficult. Management is based mainly on antibiotics wich is often probabilistic, in some cases associated with physiotherapy, percutaneous scan-guided drainage or rarely surgery.

Study: Azithromycin doubles risk for cardiovascular death versus amoxicillin
People who take the commonly prescribed antibiotic azithromycin were nearly twice as likely to die from a heart attack, stroke or other cardiovascular event than users of amoxicillin, a study found.
