CONCLUSIONS: ePWV is a promising marker for risk-stratification in ECMO patients. Frozen Elephant Trunk for Aortic Arch Reconstruction is Associated with Reduced Mortality as Compared to Conventional Techniques
To examine the perioperative outcomes following aortic arch repair using frozen elephant trunk (FET) vs. conventional elephant trunk (ET) techniques. Between 2002 and 2018, 390 patients underwent aortic repair with elephant trunk reconstruction at 9 centers: 172 patients received a FET (mean age: 65+/-13 years, 30% female, 37% aortic dissection) and 218 patients received an ET (mean age: 63+/-13 years, 37% female, 43% aortic dissection). Outcomes of interest included in-hospital mortality; stroke; and spinal cord injury (SCI).

Commentary: Caution on Causation
In this edition of The Journal, the Canadian Thoracic Aortic Collaborative (CTAC) presents a multicenter study1 comparing use of frozen elephant trunk (FET) with conventional elephant trunk (ET) techniques in aortic arch repair. The groups had similar rates of stroke (12-13%) and spinal cord injury (SCI) (2-5%), although transient SCI was higher in the FET group (2.9% vs 0%), a phenomenon that did not appear to be related to stent-graft length. The FET endoleak rate was low, with no type 1As using on-label endografts.

Right anterior mini thoracotomy approach for isolated aortic valve replacement: Early outcomes at a Canadian center
ConclusionThe sternum-sparing RAMT approach can be safely integrated into surgical practice as a minimally invasive alternative for isolated AVR, and can reduce postoperative bleeding and narcotic requirements.

First-in-Human Use of the COOK Hybrid Frozen Elephant Trunk Graft: The Canadian Experience.
CONCLUSIONS: The Cook hybrid stent-graft device provides encouraging mid-term results in a high-risk cohort. This novel graft is simple to deploy, may be customized to patients' anatomy in elective cases, eases arch reconstruction and allows versatility in the choice of arch grafts. Costs of surgical ablation of atrial fibrillation in Ontario, Canada from 2006 to 2017
ConclusionOntario taxpayers spend significant financial resources on surgical ablation of AF, a procedure lacking high ‐quality evidence demonstrating benefit in reducing mortality or stroke. Further large prospective studies examining clinically important outcomes are needed to justify its routine use in patient care and to guide allocation of healthcare funds.

Mitral valve prosthesis choice in patients < 70 years: A systematic review and meta ‐analysis of 20 219 patients
ConclusionMechanical MVR in patients aged less than 70 years is associated with a lower risk of operative mortality as well as a 20% lower risk of long ‐term death and 65% lower risk of mitral valve reoperation but 60% greater risk of bleeding compared with bioMVR in matched or adjusted data.

ConclusionsAortic valve sparing appears to be safe, and associated with reduced late mortality, thromboembolism/stroke and bleeding compared with composite valve grafting.
