Percutaneous left atrial appendage occlusion in the current practice
Kardiol Pol. 2021 Mar 5. doi: 10.33963/KP.15864. Online ahead of print.ABSTRACTOral anticoagulation (OAC) is the standard of care for stroke prevention in atrial fibrillation (AF) but is associated with a substantial risk of bleeding complications and is dependent on optimal patient´s compliance. In patients with non-valvular AF, the left atrial appendage is the source of thrombi that may cause stroke in up to 91-95% of cases. Thus, percutaneous left atrial appendage occlusion (LAAO) is increasingly being performed as an alternative to OAC for stroke prophylaxis in patients at increased bleeding risk. Current evidence sup...

Optimal anticoagulation in elderly patients with atrial fibrillation: Which drug at which dose?
Kardiol Pol. 2022 Feb 15. doi: 10.33963/KP.a2022.0046. Online ahead of print.ABSTRACTAging is an important risk factor of adverse events in elderly patients with atrial fibrillation (AF) and complicates the management of anticoagulation. Underuse of oral anticoagulants (OACs) is common in elderly patients because of comorbidities, altered physiological function of multiple organs, fragile features, risk of falls, and the lack of randomized controlled trials (RCTs) specifically for elderly patients. Nevertheless, current data still support OACs use for reducing ischemic stroke with positive net clinical benefits. Sub-analys...

Screening For Asymptomatic Atrial Fibrillation: Its Effectiveness And Cost-Effectiveness Of Repeated Ecg In Poland
Atrial fibrillation (AF) is the most common arrhythmia in humans. AF increases the risk of ischemic stroke up to 5-fold. Fortunately, this is highly preventable with appropriate oral anticoagulant (OAC) therapy. However, many patients with AF, despite the presence of risk factors, are not taking OACs, because they are not aware of the occurrence of arrhythmia prior to stroke. Therefore, it seems that the screening for AF in a broad population aimed at the earliest possible detection of asymptomatic arrhythmias, and the implementation of OAC treatment when necessary, could prevent many strokes.

Non-vitamin K antagonist oral anticoagulants (NOACs) in the treatment of coronary and peripheral atherosclerosis. Expert Consensus.
Abstract Oral anticoagulants (OAC) are widely used for prevention of systemic thromboembolism, including the reduction of the risk of stroke in patients with atrial fibrillation (AF) and prosthetic heart valves. There is also an increasing population of patients who require not only OACs, but also double antiplatelet therapy (DAPT). A typical example is a patient with AF and stable coronary artery disease or acute coronary syndrome (ACS), treated by percutaneous coronary intervention (PCI). In recent years, with the introduction of NOACs, triple or dual therapy has become safer. Regardless of these indications for...

Cost-effectiveness of a photopethysmographic procedure for screening for atrial fibrillation in 6 European countries
ConclusionThe model results showed a strong dependence of the results on the country-specific costs for stroke treatment. The use of the investigated screening method is close to cost-neutral or cost-reducing in the Western European countries and Greece. In countries with low price levels, higher cost increases due to AF screening are to be expected. Lower costs of anticoagulation, which are expected due to the upcoming patent expiry of direct anticoagulants, have a positive effect on the cost result.

Comparison of the long-term outcomes and risk factors of aortic stenosis treatment - in patients undergoing transcatheter aortic valve implantation and surgical aortic valve replacement
CONCLUSIONS: There were no significant differences regarding prognosis after TAVI and SAVR at the 24-month follow-up in propensity score matching model. Independent predictive factors of late mortality after both procedures were EuroScore II and the respiratory failure. Independent predictive factors of late mortality specific for TAVI were NOAF, increased RDW and for SAVR: TIA, stroke, COPD and RBC transfusion.PMID:35521716 | DOI:10.33963/KP.a2022.0122
