Prognostic Value of Serum Cardiac Troponin in Acute Ischemic Stroke: An Updated Systematic Review and Meta-Analysis
Ischemic stroke (IS), defined as a sudden reduction of the blood flow to the brain due to lack of blood supply to a vessel caused by thrombosis or embolism, is the second leading cause of death and third leading cause of death and disability combined worldwide.1 Stroke shares several risk factors with cardiac diseases, and approximately 25% of the patients with recent stroke had a medical history of coronary artery disease. On the other hand, cardiac adverse events are common complications after acute ischemic stroke (AIS), occurring in up to 20% of the patients during the first 2-3 days after stroke onset.

Balance of Symptomatic Pulmonary Embolism and Symptomatic Intracerebral Hemorrhage with Low-dose Anticoagulation in Recent Ischemic Stroke: A Systematic Review and Meta-analysis of Randomized Controlled Trials
Conclusions: Prophylactic/low-dose heparin increased sICH by more than they reduced sPE in patients with recent ischemic stroke. Therefore, their routine acute use cannot be recommended, but they may still be relevant in patients at very high risk of PE (eg, morbid obesity, previous venous thromboembolism, and inherited thrombophilia) or if started later, although trials have not assessed these issues.

Decompressive Hemicraniectomy for Malignant Hemispheric Stroke in the Elderly: Comparison of Outcomes between Individuals 61-70 and >70 Years of Age
Conclusions: We suggest that the efficacy of DHC in malignant hemispheric stroke patients between 61 and 70 years of age be further investigated in future randomized trials. By contrast, it appears unlikely that patients>70 years of age would benefit from DHC.

Dilemma of Indication for Thrombolysis in a Patient with Acute Ischemic Stroke Treated with a Novel Oral Anticoagulant
We report the case of a patient treated with dabigatran who developed acute ischemic stroke and was considered for thrombolysis. Because of the prolonged thrombin time (TT), thrombolysis was not performed. Repeated coagulation tests 5 days after stroke, with unchanged anticoagulant (dabigatran) treatment, found 2-fold higher TT and activated partial thromboplastin time (APTT) compared with the values found in the acute stage. Routine coagulation tests (TT and APTT) do not reliably reflect the coagulation features of a patient; therefore, there is an urgent need to develop reliable biomarkers and an adequate guideline to ...

Reduced Smoke-like Echo and Resolved Thrombus in the Left Atrium with Rivaroxaban Therapy in an Acute Cardioembolic Stroke Patient
We report a case of a nonvalvular atrial fibrillation (NVAF) patient with acute cardioembolic stroke in whom rivaroxaban, an oral direct factor Xa inhibitor, reduced a smoke-like echo in the left atrium and resolved a thrombus in the left atrial appendage. A 71-year-old man was admitted because of the sudden onset of right hemiplegia and aphasia and was diagnosed with acute cardioembolic stroke associated with NVAF. Left Atrial Appendage Morphology and Embolic Stroke of Undetermined Source: A Cross-Sectional Multicenter Pilot Study
The left atrial appendage (LAA) is the main source of thrombus in atrial fibrillation, and there is an association between non-chicken wing (NCW) LAA morphology and stroke. We hypothesized that the prevalence of NCW LAA morphology would be higher among patients with cardioembolic (CE) stroke and embolic stroke of undetermined source (ESUS) than among those with noncardioembolic stroke (NCS).

Early Magnetic Resonance Imaging Decreases Hospital Length of Stay in Patients with Ischemic Stroke
Imaging modalities are important part of stroke evaluation. Noncontrast head computed tomography (CT) is the initial imaging modality in acute stroke and although important to rule out acute hemorrhage and making a decision on thrombolytic treatment, ischemic changes may not be visible on CT for up to 24 hours. Magnetic resonance imaging (MRI) brain is an invaluable tool to confirm an ischemic stroke and facilitates stroke evaluation. Objective of this study was to investigate the correlation between time to MRI and length of hospital stay.

Encephalopathy only stroke codes (EoSC) do not result in rt-PA treatments
Isolated mental status changes as a presenting sign (EoSC+), are not uncommon stroke code triggers. As stroke alerts, they still require the same intensive resources be applied. We previously showed that EoSC+ strokes (EoSC+ Stroke+) account for 0.1 –0.2% of all codes. Whether these result in thrombolytic treatment (rt-PA), and the characteristics/ risk factor profiles of EoSC+ Stroke+ patients, have not been reported.

Tissue-Based Stroke Definition Impacts Stroke Incidence but not Ethnic Differences
In 2013, a new stroke definition was promulgated.1 Ischemic stroke was defined as an episode of neurologic dysfunction resulting from “CNS infarction.” The term CNS infarction included not only patients with clinical symptoms lasting greater than 24 h, but also those who had MRI diffusion positive signal with symptoms less than 24 h in duration. It also included retinal and spinal cord infarction as well as cerebral venous s inus thrombosis with infarction. All of these factors would increase previous estimates of ischemic stroke.

Stroke Associated with COVID-19 Vaccines
Development of safe and effective vaccines against coronavirus disease 2019 (COVID-19) remains the cornerstone of controlling this pandemic. However, there are increasing reports of various types of stroke including ischemic stroke, and hemorrhagic stroke, as well ascerebral venous sinus thrombosis (CVST) after COVID-19 vaccination. This paper aims to review reports of stroke associated with COVID-19 vaccines and provide a coherent clinical picture of this condition. Most of such patients are women under 60 years of age and who had received ChAdOx1 nCoV-19 vaccine.
