Machine Learning and the Conundrum of Stroke Risk Prediction
Stroke is a leading cause of death and permanent disability worldwide.1 Ischaemic stroke is the most common stroke variety, comprising more than 80% of strokes in the US.2 One mechanism of ischaemic stroke is atherosclerosis in the extracranial and intracranial arteries, with plaque rupture leading to thrombosis. The second major category is embolic stroke, in which thrombi form in the heart or the arterial/venous ...

Prediction of incident atrial fibrillation in community-based electronic health records: a systematic review with meta-analysis
Conclusions Models externally validated for prediction of incident AF in community-based EHR demonstrate moderate predictive ability and high risk of bias. Novel methods may provide stronger discriminative performance. Systematic review registration PROSPERO CRD42021245093.

Rivaroxaban for treatment of intraventricular thrombus in Chagas disease
We present a case of a 61-year-old man admitted for stroke 5 months after a renal embolism. An intraventricular thrombus was observed, probably the source of the cerebral and renal embolisms. The patient refused warfarin and rivaroxaban was used instead. After 40 days of treatment the thrombus had dissolved, after 20 months of regular use of rivaroxaban no more embolic events were observed. The use of rivaroxaban was effective in preventing embolic events in Chagas disease and intraventricular thrombus. <Learning objective: Warfarin is recommended for stroke prevention in patients with Chagas disease and left ventri...

First results of the Brazilian Registry of Percutaneous Left Atrial Appendage Closure
Conclusions: In this multicenter, real world registry, that included patients with NVAF and high thromboembol ic and bleeding risks, LAAC effectively prevented stroke and bleeding when compared to the expected rates based on CHA2DS2VASc and HASBLED scores for this population. Complications rate of the procedure was acceptable considering the beginning of the learning curve of most of the involved operators.Resumo Fundamento: A oclus ão percutânea do apêndice atrial esquerdo (OAAE) é uma alternativa eficaz à anticoagulação oral (ACO) para a prevenção de acidente vascular cerebral (AVC) em pacientes com fibrilação...

Successful manual thrombus aspiration in anterior ST-segment elevation myocardial infarction due to cardioembolic obstruction of the left main coronary artery
Publication date: Available online 5 December 2019Source: Journal of Cardiology CasesAuthor(s): Diego Della Riva, Matteo Bruno, Nevio TaglieriAbstractLarge clinical trials and meta-analyses have shown that thrombus aspiration (TA) in the setting of ST-T segment elevation myocardial infarction (STEMI) undergoing primary percutaneous coronary intervention (PPCI) does not improve clinical outcome, whilst it may be associated with an increased risk of stroke. Accordingly, in the most recent European Society of Cardiology guidelines the role of routine TA during PPCI has been downgraded to a class III recommendation with level ...

Non-vitamin K antagonist oral anticoagulants (NOACs) for the management of venous thromboembolism
Learning objectives Understand the importance of venous thrombosis in cardiovascular medicine. Appreciate the mode of action of different oral anticoagulants. Recall the uses, risks and benefits of each non-vitamin K antagonist oral anticoagulants. Introduction Thrombosis is the common pathophysiology responsible for ischaemic heart disease, ischaemic stroke and venous thromboembolism (VTE), and a major contributor to the global disease burden.1 This effect is markedly more pronounced by considering the view that cancer is also a thrombotic disease.2 3 Cardiovascular disease (CVD, manifesting as acute coronary syndromes, m...

Percutaneous isolation of left atrial appendage thrombus
Publication date: Available online 16 June 2017 Source:Journal of Cardiology Cases Author(s): Oh-Hyun Lee, Jae-Sun Uhm, Jung-Sun Kim, In-Jeong Cho, Geu-Ru Hong, Hui-Nam Pak, Yangsoo Jang Most physicians regard left atrial appendage (LAA) thrombus as a contraindication for LAA occlusion due to risk of distal embolization which is a serious complication. Here we report a case of successfully implanted Amplazter cardiac plug without complication in elderly patients having LAA thrombus with recurrent embolic events despite oral anticoagulants for prevention of thromboembolic events. <Learning objective: LAA thrombus is...

Thrombosis origin identification of cardioembolism and large artery atherosclerosis by distinct metabolites
CONCLUSIONS: Six metabolites as potential predictors for distinguishing between cerebral thrombi of CE and LAA origin were identified. The results are useful for understanding the pathogenesis and for secondary stroke prevention.PMID:35654581 | DOI:10.1136/neurintsurg-2022-019047

Fourth mitral valve replacement in a 15-year span in a woman of childbearing age
Publication date: Available online 13 June 2018 Source:Journal of Cardiology Cases Author(s): Laura Leticia Rodríguez-Chávez, Grecia Iveth Maryelis Raymundo-Martínez, Rodrigo Gopar-Nieto There is no report of four valve replacement surgeries in a woman during 15-year follow-up. A 23-year-old female was diagnosed at 7 years with rheumatic fever. She developed severe mitral regurgitation and at the same age a prosthetic valve was placed. In the next 15 years her mitral valve was changed 3 times because of clinical, echocardiographic, and histopathologic evidence of thrombosis. Her coagulation tests all showed infratherap...
