Walk into a typical acute hospital on a weekend or overnight, and it feels like a very different place than on a weekday. Most laboratories, theatres and diagnostics run reduced services, the corridors are quiet, and you are more likely to see a doctor or nurse rushing to an emergency than walking the wards on consults or rounds. Incidence, outcome and future projections of atrial fibrillation-related stroke and systemic embolism at age >=80 years: 10-year results of a population-based study
Conclusion Numbers of AF–related ischaemic strokes at age ≥80 years are projected to treble by 2050, along with numbers of systemic emboli, unless rates of anticoagulation in older patients increase.

Performance characteristics of methods for quantifying spontaneous intracerebral haemorrhage: data from the Efficacy of Nitric Oxide in Stroke (ENOS) trial
Conclusions Formal measurement of haemorrhage characteristics and visual estimates are reproducible. The standard ABC/2 method is superior to the modified ABC/2 method for quantifying ICH volume. Clinical trial registration ISRCTN9941422.

Recent time trends in incidence, outcome and premorbid treatment of atrial fibrillation-related stroke and other embolic vascular events: a population-based study
Conclusions We found no reduction in incidence of AF-related vascular events since publication of the BAFTA trial. A third of all disabling/fatal strokes occur in non-anticoagulated patients with known prior AF.

Aetiology, secondary prevention strategies and outcomes of ischaemic stroke despite oral anticoagulant therapy in patients with atrial fibrillation
Conclusions Stroke despite anticoagulation comprises heterogeneous aetiologies and cardioembolism despite sufficient anticoagulation is most common. While DOAC were associated with better outcomes than VKA, adding antiplatelets was linked to worse outcomes in these high-risk patients. Our findings indicate that individualised and novel preventive strategies beyond the currently available anticoagulants are needed. Trial registration number ISRCTN48292829.

Conclusions Brain volume loss precedes cognitive decline in the first year after ischaemic stroke. Tracking brain volume after stroke is important because brain volume is predictive of cognitive decline and atrophy is a hallmark of dementia.

Risk of stroke in multiple sclerosis and neuromyelitis optic spectrum disorder: a Nationwide cohort study in South Korea
Conclusions The risk of stroke is increased in patients with MS and NMOSD and seemed comparable between the two conditions. This is the first study that estimates the risk of stroke in patients with MS and NMOSD within the same population.

Hiv, antiretroviral treatment, and stroke in malawian adults
Objective We investigated HIV, its treatment and other risk factors for stroke in Malawi. Methods We performed a prospective case-control study of 222 adults with acute stroke, confirmed by MRI in 86%, and 503 population controls, frequency-matched for age, sex and place of residence. Multivariate logistic regression models were used for case–control comparisons. Whatsapp--an innovative solution for the stroke care
A 54-year-old man with a suspected stroke was admitted to a small clinical facility about 288 kilometres away from the tertiary stroke care centre with the thrombolysis facility. When the staff at the hospital contacted us, we requested them to send the CT brain images via Whatsapp on iPhone5. Whatsapp is a communication software which runs on various mobile platforms. An evaluation of acute stroke ct protocol requests made in the emergency department
Conclusions Our results suggest a greater need to educate ED physicians in recognising posterior circulation stroke symptoms and stroke mimics as well as thrombolysis guidelines.2,3 This will help avoid unnecessary ordering of CTAs particularly for patients presenting outside the thrombolysis window. This will avoid contrast related complications and also result in significant cost savings.

Fibroelastoma: a rare cause of cardioembolic stroke in the young
Conclusions Fibroelastoma is a rare but important cause of cardioembolic stroke. Often these tumours are missed on routine TTE. A high index of suspicion is needed and young patients with cardioembolic strokes should be considered for TOE and cardiac MRI. Treatment with surgical resection is warranted to reduce future risk of stroke.

Rates and correlates of suicidal ideation among stroke survivors: a meta-analysis
Conclusion Despite some limitations, this meta-analysis shows that about one out of eight stroke survivors has suicidal ideation. Thus, there is enough evidence to support the use of routine screening and early interventions to prevent and treat suicidal ideation after stroke, especially among subjects carrying specific correlates.

Prevalence of dementia in ischaemic or mixed stroke populations: systematic review and meta-analysis
An understanding of the epidemiology of poststroke dementia (PSD) is necessary to inform research, practice and policy. With increasing primary studies, a contemporary review of PSD could allow for analyses of incidence and prevalence trends. Databases were searched using a prespecified search strategy. 265 Use of support vector machine to predict discharge destination on an acute stroke ward
Conclusion From data that is available from simple assessments done in routine clinical pratice it is possible to identify patients with acute stroke for whom discharge home or inpatient rehabilitation is highly likely.
