AEDs after ICH: Preventing the prophylaxis
Has the routine use of antiepileptic drugs (AEDs) in intracerebral hemorrhage (ICH) become a habit too difficult to break? Adherents to evidence-based medicine must surely have a conniption in light of this continuing practice. The guidelines have remained clear over the years: Do not use antiseizure medications in ICH unless there has been a seizure. Yet prophylactic AED use after acute ICH remains widespread in the United States. This is brought to our attention in the current issue of Neurology®. Race/Ethnicity as a Predictor for Location of Death in Patients With Acute Neurovascular Events.

Nico launches trial for brain surgery shunt
Nico Corp said today it enrolled the 1st patient in the Enrich clinical trial which aims to explore the use of its BrainPath Approach for treating intracerebral hemorrhages. Elevated ventricular CSF lactate within the first week after aneurysmal subarachnoid hemorrhage is associated with poor outcome (P5.068)

Impact of Moderate Hyperchloremia on Clinical Outcomes in Intracerebral Hemorrhage Patients Treated With Continuous Infusion Hypertonic Saline: A Pilot Study
Conclusions: We observed higher rates of in-hospital mortality in patients who developed moderate hyperchloremia during treatment with continuous IV infusion 3% hypertonic saline, with moderate hyperchloremia independently predicting in-hospital mortality. These results suggest that chloride values should be monitored closely during hypertonic saline treatment as moderate elevations may impact outcomes in intracerebral hemorrhage patients.

Prophylactic Anticonvulsants in Intracerebral Hemorrhage
DiscussionLevetiracetam was routinely prescribed following ICH and was not associated with worse outcomes. Future investigations should examine the effect of prophylactic levetiracetam on cost and neuropsychological outcomes as well as the role of continuous EEG in identifying subclinical seizures.
