The convergent procedure: a hybrid approach for long lasting persistent atrial fibrillation ablation, the French experience.
CONCLUSIONS: Thoracoscopic hybrid epicardial-endocardial ablation technique proved to be effective and safe to treat long lasting persistent AF patients with previous failed endocardial AF ablation. PMID: 31603290 [PubMed - as supplied by publisher]

Early Hemodynamic Changes and Long-Term Outcome Of Patients With Severe Low Gradient Aortic Stenosis After Transcatheter Aortic Valve Replacement.
CONCLUSIONS: NFLG-AS was the most prevalent form of LG severe AS and was associated with adequate left ventricular compensation and good prognosis. On the other hand, CLFLG-AS represents the heart-failure with reduced ejection fraction (HFrEF) form of AS and was associated with the worst prognosis, whereas PLFLG-AS represents the heart-failure with preserved ejection fraction (HFpEF) form of AS with intermediary prognosis. Both groups showed early hemodynamic reverse response after TAVR. PMID: 31566569 [PubMed - as supplied by publisher]

Long-Term Single Centre Outcomes of Patients with Chronic Renal Dialysis Undergoing Cardiac Surgery
CONCLUSIONSCardiac surgery in DD-CRF patients is associated with high morbidity and mortality. Interestingly, overall mortality was mainly none cardiac-related, where elderly patients or those presented with CHF had the worst life expectancy. However, subsequent kidney transplantation positively impact long-term survival in such patients.

Risk factors for atrial fibrillation recurrence post Cox-maze IV performed without pre-exclusion
ConclusionsThe Cox maze IV procedure performed without pre-exclusions showed reasonable survival rates. Although AF recurred in patients with longer AFD, they fared well with significant increases in stroke volume. Concomitant atrial volume reduction may have contributed to these additional benefits.

Coronary Artery Bypass Graft Surgery Improves Survival Without Increasing the Risk of Stroke in Patients with Ischemic Heart Failure in Comparison to Percutaneous Coronary Intervention: A Meta-Analysis With 54,173 Patients
Conclusion: This meta-analysis found that CABG surgery remains the best option for patients with ischemic HF, without increase in the risk of stroke.

Survival After Myectomy for Obstructive Hypertrophic Cardiomyopathy: What Causes Late Mortality?
ConclusionsThis study, using 2 independent sources to verify vital status and cause of mortality, confirms that survival after septal myectomy is excellent. Hypertrophic cardiomyopathy was identified as the primary cause of death in less than 20%, with mortality in most patients unrelated to hypertrophic cardiomyopathy.

Transcatheter and Surgical Aortic Valve Replacement in Patients with Recent Acute Heart Failure
ConclusionsRecent AHF is associated with increased risk of mortality and morbidity after SAVR and TAVR. These findings suggest that AS patients should be referred for invasive treatment before the development of clinically evident heart failure.

Perioperative changes of the slope in the preload recruitable stroke work relationship by a single-beat technique after mitral valve surgery in functional mitral regurgitation with non-ischemic dilated cardiomyopathy
ConclusionsThe change of slope was dependent on surgical procedures. In functional mitral regurgitation, the slope may be a more sensitive parameter in reflecting the left ventricular contractile function than the left ventricular ejection fraction.

Commentary: Postoperative atrial fibrillation can last years? Oh snap!
Despite improvements in perioperative and operative care, postoperative atrial fibrillation (POAF) is still found in approximately 15% to 60% of cardiac surgical patients.1 The potential clinical impact on patients is dependent on mainly their clinical status and can include heart failure, acute renal failure, stroke or thromboembolism, anticoagulation-related complications, and even death. This is particularly true because a significant proportion of patients with aortic stenosis have diastolic dysfunction and rely on the atrial contraction to maintain adequate cardiac output.

Apical Myectomy for Patients with Hypertrophic Cardiomyopathy and Advanced Heart Failure
In this study, we describe early outcomes of patients with apical HCM following transapical myectomy and compare survival to that of patients with hypertrophic cardiomyopathy listed for heart transplantation.

Complete Sternal-Sparing HeartMate 3 Implantation: A Case Series of 10 Consecutive Patients
ConclusionsOur early outcomes suggest that the CSS approach may reduce the incidence of right ventricular failure, bleeding, intubation time, and intensive care unit length of stay. Further studies are needed to fully elucidate the advantages of CSS LVAD implantation compared with median sternotomy.
