Direct oral anticoagulants compared to vitamin K antagonist for the management of left ventricular thrombus
ConclusionsOur data suggest that DOACs are likely to be at least as effective and safe as VKA for stroke prevention in patients with LV thrombus and, despite their lack of a licence for this indication, are therefore likely to represent a reasonable and more convenient option for this setting. The optimal timing and type of anticoagulation for LV thrombus, as well as the role of screening for high ‐risk patients, should be tested in prospective, randomized trials.

Novel oral anticoagulant vs. warfarin in elderly atrial fibrillation patients with normal, mid ‐range, and reduced left ventricular ejection fraction
ConclusionsIn elderly AF patients ≥65 years, using NOAC was associated with lower IS/SE compared with warfarin in normal and mid‐range LVEF but not in reduced LVEF. Using NOACs was associated with lower death compared with warfarin in normal LVEF.

Plasma proteoglycan prolargin in diagnosis and differentiation of pulmonary arterial hypertension
ConclusionsPlasma prolargin levels differentiate PAH patients from controls and the other investigated dyspnoea groups including HF. Its potential in PAH differentiation may be enhanced by inclusion in a multi ‐marker panel. Larger studies are needed to evaluate its discriminative ability of PAH in relation to other dyspnoea aetiologies and its potential role in PAH risk stratification and pathobiology.

Characteristics, treatment, and outcomes of newly diagnosed atrial fibrillation patients with heart failure: GARFIELD ‐AF
ConclusionsPatients with AF and HF have a high risk of all ‐cause and cardiovascular mortality and stroke/systemic embolism and may develop worsening HF.

Effects of different exercise modalities on cardiac dysfunction in heart failure with preserved ejection fraction
ConclusionsIn this metabolic rat model of HFpEF at an advanced stage, Ca2+ release was impaired under baseline conditions. HIIT and MICT differentially affected Ca2+ homeostasis with positive effects of MICT on stroke volume, end ‐diastolic volume, and cellular arrhythmogenicity.

Long ‐term clinical and haemodynamic results after transcatheter annuloplasty for secondary mitral regurgitation
ConclusionsAmong high ‐risk patients undergoing transcatheter mitral valve annuloplasty for symptomatic secondary MR, mortality was ~50% at 4 years. In the surviving patients, reduced MR severity was associated with reduced NYHA functional class, reverse LV remodelling, and improved LV function.

Left atrial conduit flow rate at baseline and during exercise: an index of impaired relaxation in HFpEF patients
ConclusionsIn HFpEF, conduit flow rate decreases when diastolic dysfunction develops during exercise, in parallel with changes in LV inotropic state and torsion, contributing to impaired stroke volume reserve. Conduit flow is measurable using 3D-echocardiographic full-volume atrio-ventricular datasets, and as a marker of LV relaxation can contribute to the diagnosis of HFpEF.

Anticoagulation in cardiomyopathy: unravelling the hidden threat and challenging the threat individually
AbstractCardiomyopathy comprises a heterogeneous group of myocardial abnormalities, structural or functional in nature, in the absence of coronary artery disease and other abnormal loading conditions. These myocardial pathologies can result in premature death or disability from progressive heart failure, arrhythmia, stroke, or other embolic events. The European Cardiomyopathy Registry reports a high stroke risk in cardiomyopathy patients ranging from 2.1% to 4.5%, as well as high prevalence of atrial fibrillation ranging from 14.0% to 48.5%. There is a growing interest in evaluating the risk of thromboembolism depending on...

Impact of renin ‐angiotensin system inhibitors on long‐term clinical outcomes of patients with rheumatic heart disease
ConclusionRenin –angiotensin–aldosterone system inhibitor treatment was significantly associated with decreased risks of mortality, HF rehospitalization, and new-onset AF in RHD patients in median 5.9 year follow-up.

Safety and efficacy of iron supplementation after myocardial infarction in mice with moderate blood loss anaemia
ConclusionsIron supplementation 24  h after reperfusion of AMI is safe and reversed enlargement of end-systolic volume after AMI resulting in increased stroke volume and cardiac output. This highlights its potential as adjunctive treatment in anaemia with ID after reperfused AMI. Time point of iron application after reperfusion appe ars critical.

Mortality risk in chronic Chagas cardiomyopathy: a systematic review and meta ‐analysis
ConclusionsThe annual mortality risk in chronic Chagas cardiomyopathy is substantial and primarily attributable to cardiovascular causes. This risk significantly increases in patients with low left ventricular ejection fraction and those classified as AHA stages C and C/D.

Long ‐term impact of β‐blocker in elderly patients without myocardial infarction after percutaneous coronary intervention
ConclusionsLong-term β-blocker use was significantly associated with an increased risk of adverse cardiovascular events in elderly patients with CAD without MI or systolic HF. In particular, the chronotropic incompetence action of β-blockers could increase the risk of admission for HF in elderly patients with CAD with out MI and systolic HF, and the present findings warrant further investigation.
