This study was approved by the London—West London and GTAC Research Ethics Committee (Ref: 21/LO/0475). Informed consent will be obtained from all participants prior to data collection. The study findings will be published in peer-review journals and disseminated via conference presentations and other media. Our patient and public involvement and engagement group and ATMP stakeholder networks will be consulted to maximise dissemination and impact. Trial registration number ISRCTN80103507.

Decreased flow-mediated dilatation in patients with rheumatoid arthritis: a meta-analysis
Conclusions FMD% in patients with RA is significantly decreased compared with healthy controls. FMD% is an important early marker of atherosclerosis. It may be used as a parameter to forecast cardiovascular disease in patients with RA.

Vanishing bone disease: a manifestation of diabetic Charcot's neuropathic osteoarthropathy
A 50-year-old man with type 2 diabetes mellitus for 13 years presented with painless swelling of the left foot over the previous 18 weeks. He had earlier received multiple courses of antibiotics without improvement. There was no history of trauma, foot ulceration, rheumatoid arthritis or inflammatory arthropathy. His HbA1c was 7.6%, erythrocyte sedimentation rate 19 mm/hour, white blood cell count 9600/mm3 and uric acid 4.3 mg/dL. Examination revealed a swollen, erythematous, deformed left foot with rocker bottom deformity and loss of longitudinal, transverse arch (figure 1A). He had distal symmetric sensori...

Ochronosis
A 56-year-old gentleman had a history suggestive of inflammatory arthritis with knee joint swelling with hip and shoulder pain for 12 years. Axial symptoms of back pain and gradually developing immobility were also present. He had received a diagnosis of ankylosing spondylitis, and non-steroidal anti-inflammatory drugs with disease modifying antirheumatic drugs had been tried without much relief. He underwent surgical replacement of the knees 8 months ago, when the surgeon noticed that the cartilage was black in colour. No one in the family was similarly affected. Examination showed scleral hyperpigmented lesions...

Paeoniflorin ameliorates rheumatoid arthritis in rat models through oxidative stress, inflammation and cyclooxygenase 2.
Authors: Jia Z, He J Abstract Paeoniflorin has anti-inflammatory, anti-allergy, immune regulatory and pain-relieving effects, amongst other roles. However, the mechanisms underlying the protective effects of paeoniflorin on rheumatoid arthritis (RA) remain under investigation; the objective of the current study was to evaluate these protective effects in the context of an RA model. Rats were randomly divided into 5 groups, as follows: The control group, the RA rat model group, and the paeoniflorin groups, in which paeoniflorin was administered at concentrations of 5, 10 and 20 mg/kg for 3 weeks. The pain thresholds...

CD200/CD200R paired potent inhibitory molecules regulating immune and inflammatory responses; Part II: CD200/CD200R potential clinical applications.
Authors: Holmannová D, Kolácková M, Kondelková K, Kunes P, Krejsek J, Ctirad A Abstract SUMMARY: CD200 and its receptor were recognized as having the multiple immunoregulatory functions. Their immunoregulatory, suppressive, and tolerogenic potentials could be very effectively exploited in the treatment of many diseases, e.g. Alzheimer disease, rheumatoid arthritis, and allergy to name only some. Many research projects are aimed to develop clinically valuable methods being based on the structure and function of these paired molecules. In this review, we would like to introduce CD200/CD200R functions in a clinica...

Perifollicular haemorrhage with corkscrew hair due to scurvy
We describe a case of a man with unusual dietary habits leading to classic cutaneous manifestations of scurvy, exemplifying the typical clinical findings of this treatable disease. Case A 38-year-old Caucasian man with rheumatoid arthritis and schizophrenia presented to the emergency department with 3 weeks of progressively worsening rash, malaise and fatigue. He had no prior dermatological diagnoses and was taking no medications. For the past 6 months, due to self-diagnosed gluten insensitivity, his diet consisted solely of rice, beans, corn and carrots, and water. Physical examination revealed corkscrew (arrow)...

Republished: The association between inflammatory markers, serum lipids and the risk of cardiovascular events in patients with rheumatoid arthritis
Conclusions This study provides evidence supporting the hypothesis that RA-related systemic inflammation plays a role in determining cardiovascular risk and a complex relationship between LDL and cardiovascular risk.

Superior outcomes for military ankylosing spondylitis patients treated with anti-TNF
Discussion Prior to this study, there were no data on military AS populations receiving anti-TNF therapy. The study confirms British military patients tolerate this therapy extremely well and receive greater benefit from this treatment than that seen in any published study to date. We believe that this confirms that young age and short disease duration are good prognostic factors in the treatment of AS with anti-TNF therapy.

A transient peak of infections during onset of rheumatoid arthritis: a 10-year prospective cohort study
Conclusions We observed a marked increase in overall infections at the time of RA onset, and signs of a defective antibacterial defence mechanism, contrasting with fewer infections in the late RA stage. It can be speculated that frequent early infections initiate a compensatory immune hyper-reactivity which reduces the infection load while stimulating the development of RA in predisposed individuals.

Rat bite fever: an unusual cause of a maculopapular rash
Introduction Rat bite fever (RBF) is a rare zoonosis caused worldwide by Streptobacillus monoliformis with Spirillum minus cases restricted to Asia.1 Transmission arises mainly by scratches or bites from colonised rats.1 At-risk groups include veterinarians, sewerage and laboratory workers or rat owners.1 Patients typically present within 7 days of exposure with an acute febrile illness characterised by headache, myalgia, migratory polyarthralgia and a rash.2 In the UK, one–two cases are diagnosed per year3 predominantly caused by S monoliformis.2 It is likely underdiagnosed due to a wide differential diagnosis ...

Sarcoidosis with ocular involvement
Introduction Sarcoidosis presents with abnormal collections of chronic inflammatory cells in the lymph nodes, lungs, liver, eyes, skin or other tissues. Although first reported in 1875, it still remains a diagnostic and therapeutic challenge.1 While the condition can be asymptomatic, common symptoms include fatigue, lack of energy, weight loss, aches and pains, arthritis, dry eyes, skin lesions, swelling of the knees, blurry vision, shortness of breath and a dry, hacking cough. Ocular sarcoidosis may be noticed by accident or present before systemic disease is detected. Ocular damage caused by sarcoidosis can range from mi...

Prevalence of and risk factors for chronic arthralgia and rheumatoid-like polyarthritis more than 2 years after infection with chikungunya virus
Conclusions CHIKV is associated with a high prevalence of persistent rheumatic symptoms. Physicians need to be aware of CHIKV as a cause of acute and chronic rheumatic symptoms.
