Patterns of maternity care in English NHS hospitals
This report presents findings of a study of hospital episode statistics data which found that wide variation in practice and outcomes exists across maternity units in England. The report makes a number of recommendations including calls for units to use the indicators as a basis for reflection upon current practice and recommends that units should audit themselves against existing clinical guidelines to help identify causes of variation at a local level. Report of the Mid Staffordshire NHS Foundation Trust public inquiry
This report is the result of a public inquiry into the commissioning, supervisory and regulatory bodies in the monitoring of Mid Staffordshire hospital between January 2005 and March 2009. Following the earlier NHS inquiry in 2010, this report considers why the serious problems at the trust were not identified and acted on sooner, and identifies important lessons to be learnt for the future of patient care.
