Medication high scores
Let's have some fun. What are the highest prescribed doses of medication you've ever seen, both indicated and not indicated? Let's keep to outpatient and also things you know a pharmacy actually filled. I've had catatonic and manic inpatients on elephant doses of benzos but that's different.
Here are some of mine:
Risperidone 8mg bid. An antipsychotic was indicated, but... Not that much.

Difficult case (schizoaffective, FND, and extensive allergy list)
Hi all. So I have a difficult case that I wanted to put to the group. Options for clozapine-resistant schizophrenia?
I have a patient on the long term ward who transferred from a local short term ward, presented with persisting persecutory delusions and command hallucinations with accompanying violence risk, dx was schizophrenia. He came in on Risperidone 4mg BID, had previously been on Haloperidol, Olanzapine, Aripiprazole, and Lurasidone, Didn't want to up the Risperdal because anecdotally I haven't seen much success with doses above 8mg.

SIADH and Risperidone
I have an outpatient over 60 with schizophrenia who has had suspected SIADH associated with Risperidone. Polydipsia is not present. Hyponatremia improved with reduction in Risperidone from 5mg daily to 2mg daily, but unfortunately paranoid delusions and auditory hallucinations that were controlled on 5mg returned. Does anyone have experience with SIADH and atypical antipsychotics? A review of the literature has not been helpful so far in differentiating one atypical antipsychotic with another. Treating aggression in patients with intellectual disability?
A while ago I sat in with a patient in a psych clinic who had intellectual disability and a history of aggressive behavior. The patient's caretakers were coming in asking if anything can help with the aggression. If I remember correctly the resident prescribed Risperidone and I have no follow-up to see if that helped at all.

Unorthodox Use of Antipsychotics in Bipolar Disorder
Two questions:
1. I was reading UptoDate (Bipolar disorder in pregnant women: Treatment of mania and hypomania) and saw the following:
"The efficacy of haloperidol for treating mood elevated syndromes appears to be comparable to risperidone and olanzapine, and superior to quetiapine and lithium."
None of the attendings I work with prescribe typical antipsychotics for mania. Do you have experience doing so in non-pregnant patients? What was the result?
