Stimulant/Naltrexone for Weight
I've had some patients that benefitted from bupropion/naltrexone for weight loss. I got to wondering about my ADHD patients that are on methylphenidate or amphetamine (but not bupropion) if adding naltrexone to their regimen would be helpful for weight loss. Firing patient
How do you deal with a$$hole patients. I encountered a witch looking entitled medicaid patient today who screamed on the phone for 10 mins for not having her methylphenidate ready. Wtf?! Apparently, shes had history of bad behavior way before I started working. She threatened to transfer out. I was like HELL YES! and the she was like, i need to talk to your supervisor! Is a career of exploring potential nootropic benefits of substances that are currently regarded as illicit possible in academic neuropsychiatry??
Examples (might be completely implausible but just to give an idea of what i mean): -Pure cocaine microdosing instead of amphetamine/methylphenidate for ADHD. -LSD/Psilocybin or MDMA microdosing instead of SSRI for depression Does the government allow you to conduct studies like that? Why can't pharmacists change brand name to generic?
Today I sent over an rx for Concerta to the pharmacy. This is in Texas btw. 15 minutes later I get a phone call from the pharmacist at CVS. Pharmacist: Their insurance won't cover brand name, can you switch to generic? Me: Why are you calling me? Can't you switch it over on your own? Pharmacist: Well not really... Me: Fine then, absolutely, go ahead and change it to generic methylphenidate Pharmacist: I need your approval to switch it Me: Why? Pharmacist: I cant change it... Methylphenidate for the poor, vs non-poor
are there more to this story than what's in the mail? Medicaid ppl are getting one version of methylphenidate, and we have to save that version for them. whereas the general public gets the other crappy version of methylphenidate that poor ppl don't even want. if they're paying zero, why are they so demanding? and is it legal to save the crappy version for the general public, and the better brand for Medicaid folks "only?" what's the real story behind this discriminatory behavior?
