D. Geraint James Lecture: The sarcoidosis saga: what insights from the past will guide us in the future
Sarcoidosis Vasc Diffuse Lung Dis. 2023 Dec 20;40(4):e2023057. doi: 10.36141/svdld.v40i4.15282.ABSTRACTThe sarcoidosis community in general and the World Association of Sarcoidosis and Other Granulomatous diseases (WASOG) in particular have led efforts to improve sarcoidosis diagnosis and care. Evidence based guidelines regarding the diagnosis and treatment have recently been published. In addition, several clinical trials examining existing and new treatments for sarcoidosis have been completed and published. In addition, WASOG has developed criteria and identified Centers of Excellence for sarcoidosis care around the wor...

A case of hepatosplenic sarcoidosis accompanied by hypercalcemia
We present a case diagnosed with hepatosplenic sarcoidosis with hypercalcemia. Elevated liver enzymes and hypercalcemia were detected in the patient who was admitted to the hospital with abdominal pain and dyspnea. Abdominal dynamic magnetic resonance imaging (MRI) showed diffuse, multiple, nodular hyperintense lesions on T2 weighted. Multinuclear giant cells and lymphocytic portal inflammation were seen in the patient's liver biopsy, whose angiotensin-converting enzyme level was high, and hepatosplenic sarcoidosis was diagnosed. Shortness of breath, abdominal pain, and calcium level improved with methylprednisolone treatm...

Radiological predictive remission factors of pulmonary involvement in systemic sarcoidosis: a computed tomography scan study
CONCLUSION: In patients with systemic sarcoidosis with no lymphopenia at onset or calcifications or typical CT scan findings at presentation, we suggest a close follow-up as well as an intensive treatment.PMID:38126498 | PMC:PMC10965001 | DOI:10.36141/svdld.v40i4.15118 (Source: Sarcoidosis Vasculitis and Diffuse Lung Diseases)

Incidence, clinical features, management and outcomes of ANCA-associated vasculitis in pregnancy- a systematic literature review
Conclusions: Patients can have positive maternal and foetal outcomes following strong induction therapy, vigorous monitoring and prompt treatment of flares during pregnancy. Serious complications and flares are not associated with worse outcomes for newborns.PMID:38126499 | PMC:PMC10965006 | DOI:10.36141/svdld.v40i4.15094 (Source: Sarcoidosis Vasculitis and Diffuse Lung Diseases)

The clinical, functional, and radiological effect of long-term used immunosuppressive therapy for post-COVID-19 interstitial lung disease
CONCLUSIONS: Long-term immunosuppressive therapy may be preferred in proper cases of post-COVID-19-ILD as an effective and safe treatment option that improves the quality of life, respiratory parameters, and radiologic findings.PMID:38126500 | PMC:PMC10965009 | DOI:10.36141/svdld.v40i4.15055 (Source: Sarcoidosis Vasculitis and Diffuse Lung Diseases)

Recurrent spontaneous pneumothorax under nintedanib treatment in ınterstitial lung disease associated with systemic sclerosis
Sarcoidosis Vasc Diffuse Lung Dis. 2023 Dec 20;40(4):e2023044. doi: 10.36141/svdld.v40i4.14716.ABSTRACTInterstitial lung disease (ILD) is a common complication that can develop during the course of systemic sclerosis (SSc). Nindetanib is an antifibrotic drug approved for the treatment of systemic sclerosis-associated interstitial lung disease. Although there is an insufficient data on the development of pneumothorax, the safety of Nintedanib treatment is also uncertain. We observed recurrent resistant pneumothorax under nintedanib treatment in a patient with systemic sclerosis-associated interstitial lung disease. Nintedan...

Meta-analysis: clinical features and treatments of lung cancer in combined pulmonary fibrosis and emphysema
CONCLUSIONS: Lung cancer in CPFE, most commonly SQCC, presents in elderly heavy smokers with a male, located in the lower lobe of the lung and the areas of fibrosis predominance. Chemotherapy is the main treatment and the optimal treatment remains to be explored.PMID:38126502 | PMC:PMC10965002 | DOI:10.36141/svdld.v40i4.14433 (Source: Sarcoidosis Vasculitis and Diffuse Lung Diseases)

Quadruple overlapping autoimmune diseases manifested as sarcoidosis with mediastinal lymphadenopathy: a case report
We present a rare case of a 55-year-old female patient with quadruple overlapping autoimmune diseases, including vitiligo, autoimmune hypothyroidism, ulcerative colitis, and sarcoidosis with mediastinal lymphadenopathy. The patient had been suffering from vitiligo since 2003 and was diagnosed with autoimmune hypothyroidism and ulcerative colitis through thyroid ultrasound and colonoscopy, respectively. After physical examination, mediastinal lymph nodes enlargement was detected, and sarcoidosis was diagnosed through ultrasound bronchoscope-guided transbronchial needle aspiration (EBUS-TBNA), which is a quadruple overlappin...

Can TERT rs2853669 polymorphysm indicate fibrosis in sarcoidosis?
CONCLUSIONS: In sarcoidosis patients, telomerase rs2853669 polymorphism does not indicate progression to fibrosis, but since FEV1% was found to be lower in individuals with homozygous CC polymorphism, it is thought that it may predict loss of respiratory function. Further studies are needed to evaluate the association of telomerase polymorphisms with fibrosis in sarcoidosis.PMID:38126504 | PMC:PMC10965005 | DOI:10.36141/svdld.v40i4.14023 (Source: Sarcoidosis Vasculitis and Diffuse Lung Diseases)

An unusual case of sarcoidosis presenting as calf pain
Sarcoidosis Vasc Diffuse Lung Dis. 2023 Dec 20;40(4):e2023056. doi: 10.36141/svdld.v40i4.14004.ABSTRACTSarcoidosis is a multisystem chronic inflammatory disease predominantly affecting the lungs. Myositis as a presenting manifestation of sarcoidosis is extremely uncommon and seldom reported. Here we report a 20-year-old male who presented with bilateral calf pain for six months. On evaluation magnetic resonance imaging showed features of myositis, and muscle biopsy was suggestive of sarcoidosis with granulomatous vasculitis. Positron emission tomography-computed tomography scan revealed involvement of spleen in addition to...
