Common Pediatric Gastrointestinal Disorders
There is arguably no group of conditions more common and expansive in children than gastrointestinal disorders. Moreover, successful recognition, diagnosis, and management of these ailments is particularly challenging provided the breadth of potential dysfunction, as well as a general paucity of specific physical examination findings to pinpoint diagnoses. Elucidation of these conditions is made further challenging by frequent difficulty of pediatric patients to provide a detailed articulation of their symptoms. Nonetheless, a thorough history can aid in distinguishing these various diagnoses, which can be further classifi...

Adverse Childhood Experiences
Adverse childhood experiences are found in adults regardless of race, socioeconomic level, or education. They can be identified in a clinical environment by answering a retrospective questionnaire . Adverse childhood experiences are clearly linked to high-risk health behaviors and multiple chronic diseases. Extensive knowledge exists about the severe impact of adverse childhood experiences in adults. Identification efforts have begun for children showing the prevalence and categories of abuse. National surveillance surveys capture prevalence data for children at the state level. The for Disease Control and Prevention has d...

Pediatric Hypertension
Pediatric hypertension is becoming of increasing concern as the incidence rate increases alongside pediatric obesity. Practitioners need to be aware of the screening recommendations for early recognition and management of this disorder. Lifestyle modifications should be addressed early and specialty referral considered if the child is not improving. Further work-up to rule out secondary causes of pediatric hypertension should also be considered in any child with stage 2 hypertension and in those with persistently elevated blood pressures. Early recognition and management are key to not only preventing present complications...

Childhood Obesity
Childhood obesity is a pathologic process with multifactorial causes. The reasons range widely. Obesity leads to chronic health conditions, increasing morbidity. The management of obesity must include the patient, family, school, community, and even government for policy changes. Lifestyle changes are the mainstay of treatment, including a healthy diet and increased physical activity. Medications and bariatric surgery may have a role in certain severe cases. Community and policy changes concerning food and physical activities may facilitate practical strategies against the increasing obesity epidemic. It will help families...

Gait Disorders
As a child matures so does the child ’s gait pattern. Gait changes in pediatric patients will be expected and sequential as developmental milestones. Gait changes may also represent normal variations along an appropriate spectrum. There are times when changes in gait are due to urgent orthopedic or medical conditions, and those shoul d not be overlooked. A good understanding of pediatric gait development and a basic understanding of gait assessment are critical for the primary care physician who cares for children. In the pediatric population, asthma is the most common chronic disease. Asthma is a chronic inflammatory disease consisting of variable respiratory symptoms and airflow limitation. Proper and timely diagnosis remains of utmost importance. Early diagnosis allows for earlier treatment and subsequent reduction of morbidity and mortality. LGBTQ-Affirmative Behavioral Health Services in Primary Care
Behavioral health concerns related to sexual and gender minority stress impair functioning and limit quality of life. With greater interest in implementing LGBTQ-affirmative health care practices, primary care providers likely will see larger numbers of LGBTQ individuals presenting with behavioral health concerns. Behavioral health and medical providers may not feel prepared to address the biological, psychological, social, and cultural minority stress factors that have an impact on LGBTQ patients. LGBTQ-affirmative behavioral health providers in primary care can offer a unique service by conducting tailored evaluations an...

Lesbian, Gay, Bisexual, Transgender, and Queer Health: Translating Best Practices into Clinical Care
Within the past decade, there has been a significant increase in the education, literature, and research focusing on the lesbian, gay, bisexual, transgender, and queer (LGBTQ+) patient population. Healthy People1,2 served as the leading federal program to identify the benchmarks in medicine to improve access for the LGBTQ+ community, inform the community regarding their own health risks, and focus on prevention and measurement of patient outcomes. Understanding and Equality. Period.
I graduated medical school in 1996, at a time when deaths due to human immunodeficiency virus (HIV) disease were peaking in the United States. In addition to the “standard curriculum,” we were trying to learn all that we could about HIV and the resultant AIDS, similar to how we are struggling to quickly learn everything we can about the severe acute respiratory syndrome coronavirus 2 virus today. At that point in my medical education, I had learned a lot of medicine, but I had received no formal training on lesbian, gay, bisexual, and transgender, and queer (LGBTQ+) health, social determinants of health, how to address...
