Effect of Pain Management Electronic Order Sets on Opioid Use in Adult Rib Fracture Patients
Conclusion: Revised inpatient pain management order sets did not reduce overall opioid usage in a population of patients with 3 or more rib fractures. However, significant improvements were noted in decreased IV opioid usage on the general inpatient floors and within 24 hr of patient discharge from the hospital. (Source: Journal of Trauma Nursing)

Effectiveness of Multimodal Pain Therapy on Reducing Opioid Use in Surgical Geriatric Hip Fracture Patients
This study aimed to determine whether multimodal pain therapy could decrease opioid use without increasing pain scores in surgical geriatric hip fracture patients. Methods: This was a before-and-after cohort study. The hospital implemented multimodal pain control order sets with a standardized pain regimen and performed retrospective chart review pre- and postorder set implementation for analysis. Results: A total of 248 patients were enrolled in the study: 131 in the preorder set group and 117 in the postorder set group. The mean postoperative oral morphine equivalent (OME) was significantly lower in the postorder s...

Evaluation of Burnout and Work-Life in Surgical Trauma Acute Care Units
Conclusions: Overall, low degrees of emotional exhaustion and depersonalization were exhibited, but significant increases were noted in staff with increasing tenure. These findings suggest that cited aspects of work-life may impact the development of emotional exhaustion and depersonalization over time. Pilot interventions are underway to identify an acuity-adjusted staffing process and a user-friendly electronic documentation platform to improve the institution's work environment. Modified Need for Trauma Intervention Criteria Reduces Cribari Trauma Overtriage Rate
Introduction: The American College of Surgeons Committee on Trauma (ACS-CoT) sets standards for appropriate trauma activation criteria. Overtriage and undertriage rates are traditionally determined by the Cribari matrix using the Injury Severity Score (ISS). In 2016, the Need for Trauma Intervention (NFTI) criteria were developed by Baylor University Medical Center to overcome weaknesses in the Cribari matrix methodology. Methods: A retrospective review of trauma triage rates was conducted from March 2018 to February 2019. Overtriage rates were calculated using the Cribari matrix and then further evaluated using modif...

Timely Venous Thromboembolism Prophylaxis in Trauma: A Team Approach to Process Improvement
Venous thromboembolism is a significant complication in trauma. Multisystem injury, advancing age, surgery, and blood transfusion all contribute to the risk of venous thromboembolism in trauma patients. Our Level I trauma center was identified as an outlier with compliance in timely venous thromboembolism prophylaxis in the Michigan Trauma Quality Improvement Program, a statewide collaborative for improving trauma care. The purpose of this study was to provide an evaluation of a performance improvement project to increase the timely administration of venous thromboembolism prophylaxis in admitted trauma patients. Using a P...

The Association of Time to Palliative Medicine Consultation on Geriatric Trauma Outcomes
The integration of specialized geriatric providers with trauma services has received increased attention with promising results. Palliative medicine consultation (PMC) has been shown to reduce length of stay, improve symptom management, and clarify advance directives in the geriatric trauma population. The aim of this study was to evaluate whether PMC reduced tracheostomies and percutaneous endoscopic gastrostomies (trach/PEG) and readmission rates in the geriatric trauma population. Retrospective cohort analysis of patients 65 years of age and older, admitted to a Level I trauma center surgical intensive care unit from 20...

Trauma Data Quality Improvement: One Center's Experience With Telecommuting and Paperless Data Management
The American College of Surgeons requires that trauma centers collect and enter data into the National Trauma Data Registry in compliance with the National Trauma Data Standard. ProMedica supports employment of 4 trauma data analysts who are responsible for entering information in a timely manner, validating the data, and analyzing data to evaluate established benchmarks and support the performance improvement and patient safety process. Historically, these analysts were located on-site at ProMedica Toledo Hospital. In 2017, a proposal was developed including modifications to data collection to streamline processes, move t...

Rate of Nonsurgical Admissions at a Level 1 Trauma Center: Impact of a Trauma Nurse Practitioner Model
This study examines the impact of TNPs on the rate of nonsurgical admissions. A retrospective cohort study was conducted with 1,400 patients between January 2017 and October 2018. Two cohorts examined in this study included trauma patients whose care was managed by the TNPs versus those admitted under the care of hospitalists. The rate of admission to nonsurgical services (NSS) was 19.6% in 2017 and 13.9% in 2018, which yielded a significant decrease from previous years' percentages (p (Source: Journal of Trauma Nursing)
