An atypical case of hypomagnesemia-induced cerebellar syndrome with literature review
We present a case of HiCS presenting with a characteristic history of recurrent ataxia, tremor, and vertigo that improved with treatment. Our patient was atypical, as there were no significant MRI findings attributable to hypomagnesemia. Only seven case reports are available throughout the world that show such disparity.PMID:37936278 | DOI:10.1177/14782715231209776 (Source: Journal of the Royal College of Physicians of Edinburgh)

Adrenocorticotropic hormone-secreting phaeochromocytoma as a cause of treatment-resistant hypertension and recurrent pulmonary emboli
We report an unusual case of a patient presenting with Cushing's syndrome caused by a phaeochromocytoma secreting adrenocorticotropic hormone (ACTH). The patient had a history of treatment-resistant hypertension, secondary amenorrhoea and tendency towards hypokalaemia. She had multiple signs of Cushing's syndrome, such as swelling, bruising, abdominal striae and proximal myopathy. Hypokalaemia is more common in patients with ectopic ACTH-secretion than other causes of Cushing's syndrome. Computed tomography, adrenal vein sampling and biochemistry could confirm an ACTH-secreting phaeochromocytoma. It is important to conside...

La Valentine disease: An outbreak of exanthematic typhus in Marseille, France, in 1810
DISCUSSION: This data suggest that exanthematic typhus outbreaks among civilian populations also occurred outside the context of hospitals, in link with introduction of the disease by prisoners or soldiers.PMID:37936398 | DOI:10.1177/14782715231210333 (Source: Journal of the Royal College of Physicians of Edinburgh)

Radiation proctitis with recurrent gastrointestinal bleeding complicated by cytomegalovirus
We present the first case report of an immunocompetent 81-year-old male with multiple episodes of life-threatening GI bleeding, secondary to a non-healing CMV-positive rectal ulcer and CMV colitis following radiation for prostate cancer. Multiple insults including prostate radiation, repeated blood transfusions and CMV infection likely contributed to the recurrent bleeding episodes.PMID:37905373 | DOI:10.1177/14782715231205916 (Source: Journal of the Royal College of Physicians of Edinburgh)

Out-of-hours decision-making in deteriorating patients by foundation year doctors: Relationship to moral distress
CONCLUSION: Out-of-hours decisions by FY2 doctors were characterised by doing what is 'expected' rather than what is perceived to be 'right'. Providing guidance regarding decision-making in deteriorating patients is needed for patient safety and staff well-being.PMID:37873868 | DOI:10.1177/14782715231203941 (Source: Journal of the Royal College of Physicians of Edinburgh)
