Reducing Risks and Improving Vascular Access Outcomes
Short peripheral catheter (SPC) insertion is a common invasive procedure performed in hospitalized patients. Variations in this fundamental nursing skill exist among nurses, with significant impacts on budgets, patient satisfaction, and worker safety―key concerns for nurse leaders. A performance improvement project focused on vascular access management was undertaken, with the goal to improve SPC practice and associated outcomes. Assessment of current SPC policies, practices, products, and outcomes identified areas of potential improvement. A performance improvement program was undertaken, and its effects on clinical, sa...

Assessing and Restoring Patency in Midline Catheters
Maintaining and restoring patency in midline catheters has been a significant issue in the hospitalized patient requiring a multitude of infusates and frequent blood specimen collection. Currently, clinicians may not use this device as often as clinically indicated because they lack an approved intervention to treat thrombotic occlusions. The purpose of this research was to show evidence that alteplase, a US Food and Drug Administration–approved thrombolytic for central vascular access devices, is a safe and effective thrombolytic for midline catheters. A prospective open label study of 497 midline catheters was conducte...

Development of an Educational Protocol Based on a Nursing Team's Knowledge of Pulse Therapy in Adolescents in Brazil
The aim of this study was to assess the knowledge of nursing teams that care for hospitalized adolescents undergoing glucocorticoid infusion and to develop an educational protocol for nursing care during pulse therapy. This descriptive, exploratory study with a qualitative approach was developed in a unit specializing in adolescent health at a university hospital in the state of Rio de Janeiro, Brazil. The instrument used for data collection was a semistructured interview conducted in a private room on the unit. The main results indicated that nursing knowledge was focused on the need to evaluate hemodynamic parameters, th...

Short Peripheral Catheter Dwell Time and Associated Complications: A Systematic Review
There is evidence that the risk of complications caused by short peripheral catheters (SPCs) does not increase when SPCs are replaced due to clinical indication versus at a specific time frame. In the studies, however, the dwell time does not typically exceed an average of 3.5 days. It is uncertain how long SPCs may stay in place before there is an increased risk for complications or if there is an increased risk. This systematic review was conducted to explore the current state of the science regarding SPC dwell time as a predictor of SPC complications in adult inpatients. Implementation of a Vascular Access Experience Program to Train Unit-Based Vascular Access Champions
Vascular access team (VAT) nurses receive many consults to place short peripheral catheters (SPCs), a portion of which do not require skilled insertion by a VAT member. VAT nurses at a health care organization in the Midwestern United States developed a program to train direct care nurses as unit-based vascular access champions to increase and improve SPC placement, care, and maintenance. Twenty-one nurses from 6 units were selected to train as vascular access champions; knowledge and confidence around SPC placement were assessed before, immediately after, and 6 months after the training. The training sessions resulted in ...

The Effect of Immediate Versus Delayed Port Access on 30-Day Infection Rate
This study compared the 30-day infection risk of chest ports accessed on the same day as placement and chest ports with delayed initial access. The aim was to evaluate a larger data set that provided evidence for the development of port access guidelines. A retrospective chart review of 3322 chest port placement procedures performed between October 15, 2003, and June 10, 2015, was conducted at the interventional radiology department of a single institution. Procedure notes and health records were reviewed to determine time of initial port access, evidence of infection within a 30-day window of port placement, and causal or...
