Use of design thinking and human factors approach to improve situation awareness in the pediatric intensive care unit
AbstractBackgroundOptimal design of healthcare spaces can enhance patient care. We applied design thinking and human factors principles to optimize communication and signage on high risk patients to improve situation awareness in a new clinical space for the pediatric ICU.ObjectiveTo assess the impact of these tools in mitigating situation awareness concerns within the new clinical space. We hypothesized that implementing these design-informed tools would either maintain or improve situation awareness.Design, Settings, and ParticipantsA 15-week design thinking process was employed, involving research, ideation, and refinem...

Epidemiology of pediatric hospitalizations at general hospitals and freestanding children's hospitals in the United States: 2019 update
This study examined hospitalizations in children<18 years using the Healthcare Cost and Utilization Project's 2019 Kids' Inpatient Database, stratifying neonatal and nonneonatal hospital stays.InterventionNot applicable.Main Outcome and MeasuresSociodemographic and clinical differences between hospitalizations at general and freestanding children's hospitals were examined, applying survey weights to generate national estimates.ResultsThere were an estimated 5,263,218 pediatric hospitalizations in 2019, including 3,757,601 neonatal and 1,505,617 nonneonatal hospital stays. Overall, 88.6% (n = 4,661,288) of hospitaliz...

Sleep and circadian disruptors: Unhealthy noise and light levels for hospitalized pediatric patients
This study describes patterns of noise and light for inpatient children undergoing stem cell transplants. Objective meters tracked noise and light levels every minute for 6 months. Median overnight sound was 55  dB (equivalent to conversational speech). There were 3.4 loud noises (>80  dB) per night on average. Children spent 62% of the 24-h cycle in nonoptimal lighting, with daytime light dimmer than recommended 98% of the time. Over the 6-month period, the lowest overnight noise level recorded exceeded World Health Organization recommendations for sleep, with frequent spikes into ranges known to cause wakings. Dur...

Midline catheters in patients with advanced chronic kidney disease
This study describes current practice and acute complications associated with midlines in CKD patients.Designs, Setting, and ParticipantsTrained abstractors at 66 hospitals from the Michigan Hospital Medicine Safety (HMS) Consortium collected data on a sample of patients who received a midline during hospitalization. Patients were classified as having advanced CKD if their estimated glomerular filtration rate was<45 mL/min/1.73 m2.Main Outcome and MeasuresMidline recipients with advanced CKD were compared to those without advanced CKD by patient, provider, and device characteristics, and by the occurrence of acute compl...

Pilot study of peripheral internal jugular venous catheters on a hospitalist ‐run medicine procedure service
We describe the results of 2 years'  experience from a pilot project of a medicine procedure service placing PIJ catheters in hospitalized patients with DIVA. We successfully placed 34/35 (97%) PIJ catheters in 32 patients with zero complications. Median duration of use was 2.5 days (range 0–53 days, IQR 1–5). Catheter failure r ate within 7 days was 32.4%, though it varied across catheter types: 9.5% in 8–10 cm midline catheters versus 69.2% (p <  .001) in 6 cm angiocatheter wire introducers or shorter peripheral intravenous catheters. Our results suggest that PIJ catheters may be an option to reduce th...
