Safer health care environments by design
This article shows risk managers how evidence ‐based design is reducing medication error, staff injury, infection rates, patient falls, and more. Research knowledge can contribute to effective design solutions by simply clarifying a safety problem so solutions can be sought; it can inform the design process with potential solutions; or it can be part of a structured process where new research knowledge is created. TeamSTEPPS ®: An evidence‐based approach to reduce clinical errors threatening safety in outpatient settings: An integrative review
ConclusionsThe present review is consistent with marked improvement in communication, decrease in clinical error rates, and improvement in patient satisfaction. There is a significant need for evaluation of the impact of this validated toolkit from the Agency for Healthcare Research and Quality to maintain a sustainable health care environment for all stakeholders. Health care risk managers' consensus on the management of inappropriate behaviors among hospital staff
This study included nonprobability purposive sampling (n = 34) and three rounds of questionnaires. Consensus was reached on 8 factors: setting expectations, developing a culture of respect, holding staff accountable, enforcing a zero ‐tolerance policy, confidentiality of reporting, communicating expected behavior, open communication, and investigating inappropriate behaviors.
